Book excerpt: The purpose of this study was to investigate the effects of a fluoride releasing orthodontic primer on demineralization adjacent to brackets. Twenty-two patients were recruited for this study. One premolar was randomly chosen as the experimental tooth, the contralateral tooth was the control. Teeth were visually analyzed for white spot lesions (WSLs). Knoop microhardness was used to determine hardness. Visual examination results showed no significant difference in the number of WSLs observed between Opal Seal and Transbond XT over the duration of this study. Solid conclusions could not be drawn from the results of microhardness testing. Therefore, it cannot be concluded that there is a difference in enamel hardness between Opal Seal and Transbond XT. However, prior to 90 days, teeth showed a significant difference in WSLs. Suggesting a protective effect of Opal Seal that diminished with time. Future studies are necessary to determine the clinical performance of this product.

Book excerpt: Purpose: To evaluate the efficacy of a fluoride-containing orthodontic primer in preventing demineralization adjacent to brackets and compare the quality of enamel on tooth surfaces that received interproximal reduction (IPR). Methods: Patients at the VCU Orthodontic clinic, 10 years and older, who consented to orthodontic treatment involving extraction of at least 2 premolars were recruited to this split-mouth randomized controlled trial. Brackets were bonded to premolars using one of two primers, Opal®Seal"!or Transbond"!T. IPR was performed, and the fluoride-containing primer was applied to the randomly selected experimental tooth. Extracted teeth were analyzed visually and lesion depths were calculated using micro-CT imaging to note the presence and extent of demineralization. The prevalence and depth of lesions were compared using Fisher's Exact test and Wilcoxon Signed-Rank Test, respectively. Results: A total of 18 teeth from 6 subjects were included in the following analyses. Micro-CT analysis revealed lesions on 89% of teeth treated with Opal®Seal"!compared to 67% with Transbond"!T, but this difference was not statistically significant (p-value=0.5765). There was also no statistically significant difference between the depths of the lesions (p-value=1.00), handedness (p-value=0.5765), hygiene (0.7804), or time in the mouth (p-value=0.5601). Visual exam showed no association with treatment (p-value=1.00), handedness (p-value=1.00), hygiene (p-value=0.1373), or the time the teeth were in the mouth (p-value=0.2987). The fluoride-containing primer also seemed to have no effect on the quality of the enamel on teeth treated with IPR. Conclusion: Fluoride-containing primers may not be effective for preventing demineralization on teeth treated with orthodontic appliances.

Book excerpt: Enamel demineralization in the form of white spot lesions (WSLs) around fixed orthodontic appliances is a persistent problem in patients with poor oral hygiene. These lesions can form rapidly within 4 weeks of bracket placement. The purpose of this in-vitro study was to investigate the fluoride recharging capability ofa commercially available orthodontic primer used to minimize the development of WSLs in patients. The three groups tested were: OpalSeal (n=20, Ultradent, South Jordan, UT), ProSeal (n=20, Reliance, Itasca, IL) and Transbond XT (control, n=10, 3M Unitek, Monrovia, CA). The samples(5mmin diameter x 1mm in thickness) weresuspended individually in vials filled with 10mL of deionized water usinga fishing line. The baseline fluoride ion release from all of the samples was measured after two weeks of changing the solution every other day. The samples were then randomly divided into two groups, toothbrush or gel. The samples in the toothbrush group were brushed for one minute every day for 7d, with fluoride containing toothpaste (Colgate-Palmolive Company, New York, NY) and placed in a new solution after each brushing. After 7d of brushing the fluoride ion release was measured. The samples in the gel group were immersed in 10mL of acidulated phosphoric fluoride gel (APF) for one minute, following manufacturer0́9s instructions, and then placed in a new vial with 10mL of deionized water. At the end of 24hrs fluoride ion release measurementswere made and the samples were placed individually in a new solution. The solution was changed weekly in the gel group over six weeks to simulate the typical length of time between two orthodontic appointments. A final fluoride ion release measurement was taken of all the discs in the gel group 6 weeks after the fluoride gel treatment. The results of repeated-measures analysis indicated that there were no significant differences between the groups at baseline and after 7d of toothbrushing time points. Opal Seal exhibited a significant increase in fluoride uptake (1.0ppm) after 24hrs of fluoride gel exposure but these levels gradually decreasedover 6 weeks (0.04ppm). Pro Seal and Transbond showed no significant fluoride release after the gel or toothpaste applications. The fluoride-containing primer, Opal Seal, had the ability to be recharged with fluoride ions from APF gel. However, the amount of fluoride released from recharged discs decreased gradually over a 6 weeks of time.

To curtail this problem, amorphous calcium phosphate (ACP) containing adhesives have been developed. The purpose of this in vitro study was to evaluate the cariostatic potential of an ACP containing orthodontic bonding agent adjacent to brackets. Sixty human molars were randomly distributed into: ACP adhesive, resin modified glass ionomer cement (RMGIC), and conventional composite resin groups (N=20 each). Brackets were bonded following the manufacturer0́9s instructions. Tooth enamel through a 2mm window around the brackets was cycled in demineralization (6 hrs) and remineralization (18 hrs) solutions. After 14 days, teeth embedded in resin and were sectioned. Knoop indentations were performed to determine enamel hardness. There were no statistically significant differences between the control and experimental groups. However, both Fuji Ortho LC (RMGIC), and Aegis Ortho (ACP) showed a trend toward a reduction in demineralization. In addition, it was also shown that the initial acid etching of the enamel significantly reduces enamel hardness.
