Book The Effectiveness and Cost effectiveness of Primary Therapist Model for the Management of Patients with Rheumatoid Arthritis microform

Download or read book The Effectiveness and Cost effectiveness of Primary Therapist Model for the Management of Patients with Rheumatoid Arthritis microform written by Linda C. Li and published by National Library of Canada = Bibliothèque nationale du Canada. This book was released on 2004 with total page 392 pages. Available in PDF, EPUB and Kindle. Book excerpt: Objective. To evaluate the effectiveness and cost-effectiveness of the primary therapist model (PTM), an alternative service delivery model involving the use of multi-skilled, rheumatology trained rehabilitation therapists, for managing patients with rheumatoid arthritis (RA). Methods. The evaluation consisted of four parts. First, a systematic review was performed to examine the evidence on comprehensive physical therapy and occupational therapy for managing RA. Second, a pilot study was performed to compare the PTM with standard medical care. Third, a pragmatic RCT was conducted to compare the outcome of patients who were referred for treatment provided by a primary therapist and of those who were referred to the traditional therapy model. The primary outcome was the percentage of patients with a positive clinical response at six months, defined as a & ge;20% improvement in at least two of three measures (Health Assessment Questionnaire (HAQ) disability index, HAQ pain index, and ACREU RA Knowledge Questionnaire). Fourth, a cost-effectiveness analysis was conducted within the context of the RCT to examine the economic benefits of PTM, compared to traditional therapy (TT), from the perspectives of society (base case) and the health care system. Sensitivity analyses were performed over various approaches for costing lost productivity. Results. Of the 144 consenting patients, 73 were assigned to the PTM group and 71 to the TT group. A total of 111 patients (PTM = 63; TT = 48) completed the required assessments and were included in the analysis. At six months, 44.4% of patients in the PTM group met the clinical response criterion versus 18.8% in the TT group (chi2 = 8.09, df = 1, p = 0.004). The base-case economic evaluation showed that the use of PTM resulted in a cost-utility of $13,675 per QALY gained compared to the TT. From the perspective of Ministry of Health, the PTM dominated the TT, with an overall annual cost saving of $1,060 per patient. Conclusion. Patients who were referred to receive treatment for RA from a primary therapist might achieve better outcomes than those who were referred to traditional therapy. The results, however, should be interpreted with caution due to the high rate of dropout in the TT group.

Book Psychosocial Factors in Arthritis

Download or read book Psychosocial Factors in Arthritis written by Perry M. Nicassio and published by Springer. This book was released on 2015-11-27 with total page 298 pages. Available in PDF, EPUB and Kindle. Book excerpt: This book provides an integrative approach for the management and care of the arthritis patient. Many different psychological, social, and behavioral factors have been shown to affect the quality of life of arthritis patients beyond the effects of the disease and the response to medications and treatments. The book establishes a biopsychosocial framework for understanding the interplay between psychological and physical aspects of rheumatic disease and summarizes the research about the psychosocial aspects that affect patients with arthritis, including stress, social support, doctor-patient communication, and sleep patterns. It explores the practice of incorporating psychological and behavioral approaches in the rheumatology clinic in order to surmount the problems of access and enhance the quality of care, promotes a comprehensive perspective on the numerous issues affecting persons with arthritis, and provides clinical guidelines for management. Psychosocial Factors in Arthritis is a valuable resource for behavioral medicine researchers, behavioral clinicians, and a range of arthritis professionals, including rheumatologists, nurses, and other allied health service practitioners.

Book Patient initiated Care and Self monitoring in Rheumatoid Arthritis

Download or read book Patient initiated Care and Self monitoring in Rheumatoid Arthritis written by and published by . This book was released on 2023 with total page 0 pages. Available in PDF, EPUB and Kindle. Book excerpt: The hypothesis of this thesis was that patient initiated care (PIC) with self-monitoring symptoms in an app would safely reduce costs and save time in patients with rheumatoid arthritis (RA). Furthermore, the thesis describes the development of the app and its feasibility, explores (facilitators and barriers to) ePRO completion/adherence and the relationship between ePRO results and clinical parameters. Our results show that while PIC with self-monitoring is safe and reduces the number of visits, it does not lead to lower costs from a societal perspective. Despite not being of lower costs, the intervention, and PIC interventions in general, might still be useful. We could consider implementation of the intervention as it is of similar cost as usual care, and requires less consumption of time for rheumatic healthcare providers and patients. This will be increasingly important in the coming years due to the growing shortage of healthcare personnel.(5,6) However, the number of visits with other healthcare providers following the intervention should be monitored, as there was a trend to visit other healthcare providers more often following the intervention. Furthermore, the intervention might be of particular interest to regions/countries where there are long(er) waiting lists and more rural areas with a high average travel time to rheumatology clinics. This thesis also shows that self-monitoring is feasible, and we have identified several barriers and facilitators to ePRO completion that could improve adherence to self-monitoring. Rheumatologists should discuss results and allow patients to skip visits when the scores are low (to maximize adherence). With focus groups, we should evaluate why not all rheumatologist discuss results, so that their barriers for discussing results can be understood and possibly taken away with specific instructions that address their hesitations (which we do not know yet).

Book Comprehensive Occupational Therapy for Rheumatoid Arthritis

Download or read book Comprehensive Occupational Therapy for Rheumatoid Arthritis written by Carin Dreijer du Plessis and published by LAP Lambert Academic Publishing. This book was released on 2012 with total page 148 pages. Available in PDF, EPUB and Kindle. Book excerpt: Rheumatoid Arthritis (RA) is a chronic disabling joint disease. Empowering patients with RA to self manage the disease can lead to changes in behaviour, pain or physical health and psychosocial health status. Limited evidence exists for the efficacy of comprehensive Occupational Therapy (OT) intervention for patients with RA in improving functional ability. This study aimed to evaluate the impact of comprehensive OT intervention on patients with RA in producing a sustained improvement in patients' functional ability. It was found that a comprehensive in-patient occupational therapy programme does lead to sustained significant improvement in functional ability and Health related quality of life (HR-QOL). Focus groups and home based intervention appears to have a clinically significant impact on disease activity.
