Book excerpt: The purpose of this study was to compare metabolic and performance effects of ingestion of glucose, acid-modified high amylose starch and placebo during endurance exercise at 60% of V02 peak. Specifically, we sought to determine effects on plasma glucose maintenance, total carbohydrate oxidation as measured by indirect calorimetry, exogenous carbohydrate oxidation using 13C02 excretion, and endurance performance. Ten male subjects cycled to fatigue at 60% of V02peak, after ingesting acid-modified high amylose starch (AMS), glucose, or a sweet non-caloric placebo. The ingestion protocol consisted of a bolus feeding of 4 ml/kg (280 ml, 22 g CHO for a 70 kg person) at the onset of exercise, and 2 ml/kg body weight every 15 minutes for the duration of the exercise. All trials were separated by 1 week and treatments were administered in a random, double blind manner. The plasma glucose derived from exogenous carbohydrates (CHO) at the time of fatigue for AMS, and glucose were 23% and 46% respectively. A significant difference was seen among treatments. Maximum exogenous 13C oxidation rates were 0.69 g/min, 0.21 g/min for the GLU and AMS trials respectively. Exogenous carbohydrate oxidized was different among treatments. Overall time to exhaustion was significantly lower in the CON treatments (138 min), compared to the AMS (158 min) and GLU (159 min) treatments. Carbohydrate in the form of an acid-modified starch was digested, absorbed, and metabolized when ingestion during endurance exercise, as supported by the direct measured of absorption and oxidation of the exogenous carbohydrate using 13C methodology.

The use of carbohydrates ingested either before exercise or during has been a topic of numerous studies. However previous studies have been seriously limited by the use of non-athletically representative outcome measures. This study was designed to test whether or not ingesting a carbohydrate solution 30 min prior to endurance exercise would have an effect on endurance performance. Ten well-trained cyclists performed three 40 km time trials (one for habituation purposes), using a control and a carbohydrate solution (35 grams). Time to completion, power output, velocity, blood lactate, blood glucose (BG), HR, and RPE were measured during the ride. There were no significant differences, except for a significant drop in BG at the beginning of the ride. Based on these results, the use of carbohydrates before endurance exercise does not hinder endurance performance.

The use of a low carbohydrate (7% of calories)/high fat (74% of calories) diet (LCD) has been suggested as a means of improving endurance during moderate intensity exercise. Little is known about the interactions between preexercise eating and its effects on mechanisms of cardiovascular and metabolic control. A series of experiments were performed to establish a basis for manipulation of the metabolic system to improve exercise performance. Preexercise feeding resulted in reduced skeletal and cardiac muscle blood flows at identical exercise intensities in spite of an increased cardiac output needed for the digestive process. The finding of reduced myocardial blood flow, accompanying the increased blood flow demands for the digestive process during exercise has implications related to the occurrence of postprandial angina in man. The interactions between adaptation to a LCD and preexercise eating have potential application to enhancing performance of combat personnel. The interrelationships among these and other variables are briefly discusses.

Book excerpt: It is well documented that consuming carbohydrates (CHO) prior to exercise has been shown to alter metabolism. There are many ways that CHO ingestion affects substrate utilization and blood glucose dynamics at the start of exercise. Changes in the concentrations of blood glucose, insulin, glucagon, free fatty acids (FFAs) as well as varying utilizations of different substrates have been observed. Each of these responses is reflective of the body’s capacity to maintain homeostasis through different physiological conditions and demands. PURPOSE: The aim is to deduce how varying amounts (approximately 0, 12.5, 25 and 50g for a 70 kg person) of pre-exercise sucrose ingestion effects metabolism including blood glucose concentration and fat oxidation during 30 minutes of moderate intense exercise. METHODS: This will be a randomized crossover study. After the initial assessment of baseline data (VO2peak), participants will be asked to perform four cycling trials at 50% of VO2peak for 30 minutes. Forty-five minutes before each exercise trial, participants will consume 0, 12.5, 25 and 50g (for a 70kg person) of sucrose. VO2 and VCO2 will be collected for 15 minutes prior to exercise and for the entire 30 minutes of cycling. Blood glucose will be obtained through the finger prick method and collected directly prior to exercise, 5, 15 and 30 minutes into cycling. Heart rate and rate of perceived exertion will be measured every 5 minutes of exercise. DISCUSSION: It is speculated that a small dose (12.5g for a 70kg person) of pre-exercise sucrose consumption will be able to demonstrate a decline in blood glucose concentration during exercise with a step-wise reduction in fat oxidation. This dose response curve will display the sensitivity of metabolism to ingested sucrose.
