Book excerpt: Purpose: The purpose of this investigation was to evaluate the effect of mechanical vibration on pain experienced and rate of tooth movement (OTM) during the initial alignment phase of orthodontic treatment. Materials and Methods: Participants were randomly allocated to control, low frequency or high frequency. An electric toothbrush was modified to provide high frequency vibration. A low frequency vibration device was acquired from a commercial vendor. Experimental groups applied vibration to the dentition for 20 minutes daily. All participants completed questionnaires using visual analog scales at five time points. Pain experience was measured in millimeters (mm) with the scale measuring 100 mm in length, left hand side at 0 mm. Rate of alignment was assessed using an Irregularity Index of mandibular incisors correlated to days until full alignment of the lower arch. Results: High frequency vibration participants reported significantly less pain at 4 hours compared to low frequency participants (p=0.0460). No other significant differences for pain were found. Participants who received low frequency vibration with a higher initial irregularity aligned significantly slower than control participants (p=0.0107). No other significant differences in rate of alignment were found. Conclusions: Mechanical vibration significantly reduced the pain experience for the high frequency vibration participants compared to low frequency vibration participants at one of five time points (4 hours) during the first week of orthodontic treatment. Mean alignment rates did not significantly differ between treatment groups. Participants who received low frequency vibration with high initial irregularity aligned significantly slower compared to control participants.

Book excerpt: Background: Supplemental vibration has been introduced to orthodontics to accelerate the rate of tooth movement and to reduce patient discomfort. Purpose: This study investigated the impact of the AcceleDent® device on Invisalign® treatment. Specifically, we wished to determine if using the AcceleDent® device allowed aligners to be changed with a one-week, rather than the usual two-week, regimen. Design: This 2-armed, randomized, triple-blinded, active-controlled clinical trial was carried out in 2 orthodontic practices in Seattle, WA and Vancouver, BC with a 1:1 allocation ratio. 26 adult subjects (12 male and 14 female, mean age=33) were randomly allocated to either an active or control AcceleDent® device. The control devices were exactly like the active devices, but the coupler that transmitted the force to the mouthpiece was removed. All patients were placed on a one-week regimen for changing aligners and were evaluated by their orthodontist every three weeks. If the fit was adequate, the patients would continue with treatment. If there was a lack of fit, based on pre-established guidelines, the patient would be considered a non-completer. The primary outcome compared the percentage of completers in the active and control groups. A secondary outcome measured the final incisor irregularity and the change in incisor irregularity for those who completed their regimen of aligners. Results: Fisher’s exact test showed no significant difference in completion rates between the two groups (Active = 77% completion, Control = 85% completion, p=.99). Independent-sample t-tests showed no significant difference between final incisor irregularity (p=0.75) or the change in incisor irregularity (p=0.74) between the two groups. Compliance with aligners and the AcceleDent® device were similar in both groups. Conclusions: This study found no evidence that AcceleDent® impacts the ability to complete a series of aligners with a one-week change regimen or the final alignment achieved in this adult patient. Book excerpt: The rationale behind this study is to detect the effect of obesity and/or orthodontic treatment on the levels of the selected biomarkers in different bio-fluids. The second study is a prospective cohort study in which data were collected from 12-18 years old normal weight and obese patients at 4 time-points: (T1) prior to treatment at the normal records appointment; (T2) 1 hour following placement of the fixed appliance; (T3) 1 week following placement of the fixed appliance; and (T4) at the end of alignment stage (0.019 x 0.025 - inch stainless steel archwire in the lower arch). The responses to orthodontic treatment were assessed in different ways: (1) Rate of tooth movement using dental study casts (T1, T3, T4); (2) Pain and discomfort using a self-reporting questionnaire (1st week); and (3) Biochemical assay of markers in UWMS, GCF and peripheral serum (T1, T2, T3, T4). The data of this study presented that the rate of OTM was significantly higher in obese patients compared to normal weight, and obese patients needed less time to achieve tooth alignment compared to normal weight, but this was non-significant. Obese patients experience higher mean pain than normal weight patients accompanied by higher consumption of analgesics. Alongside, GCF was more likely to express biochemical changes during OTM compared to UWMS and serum, with GCF-levels of leptin; resistin, MPO and RANKL were significantly different between obese and normal weight patients and associated with observed rates of OTM.

Book excerpt: The purpose of this study was to gather longitudinal data to quantify and describe the pain experience (intensity and quality) and rates of tooth movement during controlled human tooth movement, specifically where stresses are changed from relatively light (4 kPa) to heavy (52 kPa) and heavy to light. In a split mouth design, maxillary first premolars were extracted, posterior anchorage appliances were placed, and canines distalized using custom vertical loops activated by nickel titanium springs calibrated to deliver 4 or 52 kPa. Subjects were seen for 11 visits over 84 days. The study period was split into 4 phases: Day -35 to Day 0, Day 1 to Day 3, Day 7 to Day 35, and Day 42 to Day 84. Subjects were blinded to the stress levels and at each study visit completed a validated pain report for right and left canines consisting of the Visual Analogue Scale and the Modified McGill Pain Questionnaire Short Form designed for orthodontic subjects, plus had maxillary impressions made for analysis of tooth movement. For a given stress, Friedman and Wilcoxon Signed Ranks tests were used to evaluate for significant effects of phase on pain score. After testing for equal variances, two independent t-tests were employed to investigate whether or not there were significant differences between teeth moved by 4 versus 52 kPa for Visual Analogue Scale (VAS) and rate of Orthodontic Tooth Movement (OTM). Pearson Correlation analyses determined relations between VAS and rate of OTM by phases. Subjects reported slightly elevated pain associated with canines moved by 52 kPa compared to 4 kPa during Phase 2, however, there were no statistically significant (p = 0.279) differences in pain intensities and qualities between the two levels of controlled stresses. In Phase 4, teeth exposed to 52 kPa moved on average at higher rates of movement than teeth exposed to 4 kPa. However, these rate differences were not statistically significant. There were no significant correlations between subject reported pain responses and rates of tooth movement.

Reducing treatment time decreases risk of caries, periodontal disease, root resorption, and increases patient satisfaction. Orthodontic tooth movement (OTM) is a result of bone resorption and apposition; factors influencing the rate at which these processes take place may affect OTM. Various attempts have been made to shorten treatment time. The regional acceleratory phenomenon (RAP) has been used to modify the balance between resorption and apposition of bone through selectively injuring the cortical plate of the alveolus to accelerate tooth movement. This approach is limited due to the need of flap reflection affecting the acceptance of this treatment modality by patients due to the invasive nature of the procedure. In order to induce RAP without flap reflection a new approach called corticision has been introduced, which consists of transmucosal manipulation of the alveolar bone with a scalpel incision without flap reflection. Since any manipulation of alveolar bone can potentially increase RAP and hence accelerate bone remodeling, we hypothesize that corticision will increase the rate of orthodontic tooth movement. In order to test this hypothesis we will use an in vivo rat orthodontic tooth movement model.
