Book excerpt: Breastfeeding provides many health benefits for both mother and baby, and mothers need support and encouragement in order to have breastfeeding success. National initiatives to promote breastfeeding support have been implemented in an effort to improve breastfeeding outcomes. The breastfeeding support that mothers receive should come from communities, healthcare providers, and nursing students. However, researchers reported that nursing students are not receiving the educational preparation to provide breastfeeding support that mothers need. The purpose of this study was to determine if an evidence-based educational intervention would have an effect on baccalaureate nursing students' knowledge and attitude in regard to breastfeeding support provided for mothers. A randomized pretest-posttest with comparison group design was used and breastfeeding knowledge and attitude scores were evaluated utilizing the Australian Breastfeeding Knowledge and Attitude Questionnaire (ABKAQ). Students participated in an evidence-based breastfeeding lecture followed by participation in either a simulation role-play with a standardized patient for the experimental group or viewing of an educational breastfeeding video for the control group. Statistical analysis results revealed a significant difference in pretest and posttest scores in regard to the students' breastfeeding knowledge and attitudes toward breastfeeding. No significant difference was found in breastfeeding knowledge and attitudes toward breastfeeding pretest and posttest scores between the groups. Although the results of this study revealed that there were no differences in breastfeeding knowledge and attitudes toward breastfeeding between the experimental and control groups, providing students with the evidence-based educational interventions was beneficial as increased breastfeeding knowledge and attitudes toward breastfeeding were achieved.

Book excerpt: Background: Breastfeeding is recognized as the optimal source of nutrition for infants during the first six months of life. There are numerous health benefits from breastfeeding for both mothers and infants. Nurses are in a position to provide breastfeeding education and hands-on support in the immediate postpartum period, helping to set patients up for a successful breastfeeding journey. The goal of this project using Nola Pender’s Health Promotion Model was to determine if an educational intervention utilizing breastfeeding resources from the Association of Women’s Health, Obstetric, and Neonatal Nurses increased nurses’ knowledge level and confidence providing breastfeeding education. Methods: An educational intervention with pretest and posttest design for comparison was used. Results were compared and analyzed using descriptive statistics. Results: 22 obstetric nurses (N=22) completed pretests and received breastfeeding education. 17 posttests were returned (return rate 77.3%). Pretest scores revealed a mean score of 67%. Posttest results revealed a mean score of 88.2%. Nurse confidence providing breastfeeding support prior to the educational intervention averaged 4.09 (out of five). Nurse confidence providing breastfeeding support post-intervention increased to an average of 4.53 (out of five). These results demonstrate the significance of improvement in breastfeeding knowledge and confidence providing breastfeeding support following an educational intervention regarding breastfeeding. Conclusion: The education intervention was successful in improving obstetric nurse breastfeeding knowledge and confidence providing breastfeeding support to patients. It is recommended based on the evidence that obstetric nurses undergo regular breastfeeding education to maintain current knowledge and build confidence providing breastfeeding support to patients. -- Abstract.

Abstract: Breastmilk is the most natural and complete nutrition for infants, and breastfeeding incurs numerous benefits for both infants and mothers. The World Health Organization recommends that mothers exclusively breastfeed their infants for six months and continue breastfeeding for up to two years of age and beyond. More than 80% of Hong Kong mothers now initiate breastfeeding. However, overall breastfeeding duration remains short and rates of exclusive breastfeeding are low. Premature weaning is influenced by multiple factors, and mothers require additional support from healthcare professionals to overcome various lactation difficulties in the early postpartum period. This study aimed to evaluate the effect of two postnatal professional support interventions on the duration of any and exclusive breastfeeding. We conducted a cluster randomized controlled trial to evaluate the efficacy of in-hospital support and post-discharge telephone support on breastfeeding outcomes among first-time mothers in Hong Kong. Seven hundred and twenty two mother-infant pairs with uncomplicated, full-term pregnancies were recruited from the postnatal units of three large public hospitals. With the hospital as the unit of randomization, participants were allocated to standard care (n=263), in-hospital support (n=191), or post-discharge telephone support (n=268). Mothers in the in-hospital group were given three individualized breastfeeding support sessions during the postnatal hospitalization, with two sessions given in first 24 hours after birth and one the following day, providing lactation knowledge and instructions on breastfeeding techniques. Participants in the telephone group were provided with weekly 30-minute breastfeeding counseling and support sessions for the first four weeks after delivery, with first contact initiated within 72 hours after discharge. The primary study outcomes were the prevalence of any and exclusive breastfeeding at one, two and three months postpartum. Seven hundred (97%) participants completed the six-month follow-up, while eleven had partial follow-up and eleven could not be contacted. The rates of any and exclusive breastfeeding were higher among participants in the two intervention groups at all follow-up points when compared with those who received standard care. Participants receiving the telephone support were significantly more likely to continue any breastfeeding at one month (76.2% vs. 67.3%; OR=1.63, 95% CI 1.10-2.41) and two months (58.6% vs. 48.9%; OR=1.48, 95% CI 1.04-2.10), and to be exclusively breastfeeding at one month (28.4% vs. 16.9%; OR=1.90, 95% CI 1.24-2.91). Participants in the in-hospital support group were also more likely to be breastfeeding at all time points in the first six months but the effect was not statistically significant. Early professional breastfeeding support, especially weekly telephone counseling, significantly increased the rates of any and exclusive breastfeeding in the early postpartum period and significantly increased overall breastfeeding duration in the first six months among first-time mothers. Additional postnatal professional support when given to breastfeeding mothers on a one-to-one basis can improve breastfeeding outcomes. Book excerpt: Problem: Breastfeeding has long been recognized as the preferred method of feeding in the first year of life (American Academy of Pediatrics, 1997). Extensive research confirms the nutritional, economic, biomedical, immunological, and psychological advantages of breast milk. Despite the clear benefits of breastfeeding to mother and infant, breastfeeding rates today continue to remain below the recommended levels in the United States (US), most notably among low-income mothers. Research has shown that breastfeeding self-efficacy is one predictive factor affecting breastfeeding duration. Theoretical Framework: Dennis's (1999) Breastfeeding Self-Efficacy Theory, was the basis for this study. Using this theory, the Breastfeeding Self-Efficacy Intervention Program (BSEIP) was developed. The intervention consisted of a one-hour program delivered during the last trimester of pregnancy. The BSEIP consisted of measures to increase breastfeeding self-efficacy, including education, practice, and demonstration of breastfeeding techniques. Social support and practical advice were provided in the first two-weeks of the postpartum period. Methods: A quasi-Experimental design was used to test the effect of the BSEIP on duration of breastfeeding. A convenience sample of 36 low-income predominately non-Hispanic White women was recruited from two prenatal clinics on the outskirts of a large metropolitan area. All women indicated their intent to breastfeed their infant. Women were assigned by prenatal clinic to either receive or not receive the BSEIP. Data were collected using the Breastfeeding Self-Efficacy Scale (BSES) and a demographic profile. Women were contacted by telephone at two and six weeks postpartum to determine if they were still breastfeeding and to complete the BSES. Results: Women who received the BSEIP had greater breastfeeding self-efficacy at two and six weeks postpartum than women who did not receive the intervention; women who received the intervention also increased their self-efficacy scores over time. The mean duration of breastfeeding between the two Groups was also statistically significant: Women who received the intervention breastfed for an average of 28.82 days compared to 11.86 days for women who did not receive the intervention. Implications: The results of this study suggest that the one-hour BSEIP during the last trimester of the prenatal period may increase the duration of breastfeeding in low-income women who intend to breastfeed. This study supports the literature which found that prenatal education and postpartum support are important to the outcome of breastfeeding.
