Book excerpt: Excerpt from The Diagnosis, Pathology, and Treatment of the Diseases of the Chest To supply the wants of his class, and of others who felt an interest in the subject, the author published, in the year 1835, a short treatise on the Diagnosis of Thoracic Diseases, embodying what was most im portant in reference to the physical and general signs; this manual did not include the treatment, and was found upon experience to be less complete than was desirable. At the request of many of his pupils, he published a series of lectures during the years 1840 - 1, in the Medi cal Examiner, which, with some additional lectures, were collected into the first edition of the present work. The form of lectures in the first edition was retained as the most convenient; some of them were condensed by a careful hand from the oral lectures of the author; the greater part, however, were written by himself, forming the substance of the course. They necessarily were less full than the lectures as delivered, and the illustrations were of course excluded; but they comprised the most important parts of the subject, such as are most worthy of being remembered, and seem essential to a knowledge of the subject. In the present edition the work is divided into the usual form of chapters, arranged according to the subjects of which they treat. Book excerpt: Excerpt from Thoracic Diseases: Their Pathology, Diagnosis and Treatment One object which Dr. Newton had in the preparation of this work, was to supply the increasing demand in the New School of Medicine, for a scientific treatise upon the Pathology, Diagnosis and Treatment of Thoracic Diseases; and another was to make public the results of his own study and investigation into General and Special Pathology, and the means of Physical Diagnosis. Many new remedies, though in general use among the physicians of the New School, yet not in common use by the whole profession, are substituted for those upon which dependence has chiefly been placed for the removal of inflammatory diseases. That they are much more efficient, and at the same time less injurious to the constitution, a thorough trial will demonstrate. During nine years, Dr. Newton had given special attention to the study of thoracic diseases, and their treatment, and hence he could reasonably claim ample qualification to execute the task which he commenced. He had formed the plan for the whole work and had written all the general principles of pathology and diagnosis, and also, a particular description of several of the more important diseases of the thorax. Dr. Newton's writing ends on the two hundred and twenty-sixth page, at which place the writing of the Completing Author commences. In the completion of the work, the pathology, diagnosis and prognosis, have been chiefly derived from the best medical authorities of the age; and yet, such alterations have been made as investigation seemed to suggest. The treatment recommended, is substantially that of New School Authors, with such modifications as have seemed necessary, and of practical utility. Being a student of Dr. Newton in 1847 and 1848, and having been since that time associated with him in the practice and teaching of medicine during a limited period, good opportunities have been afforded for learning his peculiar views of the pathology and treatment of disease. From many other medical gentlemen of extensive experience much valuable information has been derived, to whom the Completing Author would here express his thanks for the interest which they have manifested in the work, and for their many voluntary contributions to the treatment of disease. In conclusion he would simply say, that he has used every possible effort to make the work valuable for the profession, to which it is now offered, with the hope that it maybe the means of alleviating human suffering, and of the advancement of sound medical education. We have attempted to omit everything not of practical diagnostic use, and to condense methods of secondary importance. More than is the custom has been written on the subject of diagnostic acoustics, because we believe that only through the comprehension of the laws of sound production and transmission can the results of percussion and auscultation be intelligently interpreted. We have endeavored to teach as much as possible by means of illustrations. Of these, many are photographs of frozen sections from the cadaver, previously hardened in formalin, so that the anatomic relations of the tissues remain as during life. We have found these specimens invaluable in our own teaching; and the book has been written in the hope that the photographic reproductions of our sections may be useful to others. We take pleasure in acknowledging our great indebtedness to Dr. George Fetterolf for the frozen sections; to Dr. J. In the pages which treat of acoustics, I have attempted to collect, in a simple and intelligent form, the principles upon which the physical diagnosis of disease within the chest is based, freely illustrating them with examples, in order more closely to connect the theories of the science with the practice of the art. The succeeding chapters are principally occupied with dissertations on the plates, and directions for educating the organs of sense to appreciate the physical signs of disease.
