At the same time, he sees these institutions from without and from the standpoint of an active general practitioner. I feel sure that the book will be of use to many physicians. Book excerpt: Excerpt from Tuberculin in Diagnosis and Treatment The use of tuberculin in diagnosis and treatment has received wide attention during the past ten years. New methods of application in diagnosis have stimulated a keen interest to determine their value, and recent changes in attitude toward important questions bearing upon immunity have provoked extensive researches, undertaken to make clear its mode of action. Tuberculin treatment, sternly rejected after its unsuccessful introduction, has again become widely popular and discussion is waged as to its true value, the most desirable preparation and the best methods of administration. A voluminous literature has grown up, and from these innumerable contributions we are now in a position to filter off certain facts that have crystallized. About much we are still uncertain, but there is enough that is definite to permit us to take bearings for our practical course. While this literature is easily accessible, it is widely distributed and a large part of the best of it is in foreign languages. No satisfactory summary has appeared in English, and such a summary seems particularly desirable, as overstatements and misconceptions incident to early enthusiasm have stuck fast in the minds of many and have remained uncorrected by the overwhelming evidence of later investigations. In our experience there is a very general lack of knowledge of precisely what a reaction to tuberculin means, and of what account is the information gained from its use when applied to a particular instance. Numerous cases are still sent to us with a diagnosis of pulmonary tuberculosis based solely upon a positive cutaneous reaction, and in medical discussion one hears the most bizarre interpretations of the subcutaneous test. During the past five years we have used tuberculin largely, both in diagnosis and treatment, at the Phipps Dispensary of the Johns Hopkins Hospital, and the experience we have gained is made the basis of this presentation. We not infrequently have what we might term a secondary reaction coming on usually from three to five days after a dose has been given. This usually manifests itself by a slight temperature reaction and is sometimes accompanied by some of the slighter symptoms mentioned above. This is probably due to an autoinoculation of bacterial products from bacilli which have been destroyed as a result of the previous injection. That this explanation is probably correct may be inferred from the fact that if another injection of tuberculin be given at this time an increase of the reaction symptoms is apt to occur, showing that the dosage plus the amount of bacterial products of the autoinoculation taken together is too great a dose for the patient at the time. A dose should not be administered when such a secondary reaction is present. That this secondary reaction is not due to the "mobilization of bacilli" is proven by the fact that the temperature disappears at once. In fact, this "mobilization of bacilli" which was talked of so much by Virchow's followers is a myth when the remedies are used properly. I have given many thousands of injections and have never seen it occur. Moeller (Therapie der Lungenschwindsucht, Schroeder und Blumenfeld, page 246) also says that he has never seen it occur in 28,000 injections that he has given. There has been some discussion on this point, and like many Tuberculin be otners k still an open question. The objection to the use Used on a of tuberculin at such a time is based upon the belief that the Fever1? rise of temperature is due to an autointoxication due to tuber cle toxins being set free and being poured into the blood stream from the tuberculous focus. I have endeavored to show the importance of local treatment in the early stage of the affection, particularly in the chronic form, especially where follicular degeneration has preceded the development of tubercles in the lungs. Great pains were taken in the examination of the cases of phthisis which presented themselves for treatment. Of 558 cases of phthisis that came under my observation, 311 were males and 247 females. Of this number, 377 had disease in the right lung, and 234 in the left. Of the females (247) 147 were first diseased in the right lung, and 100 in the left. In the treatment of a large number of cases during many years of observation, about the same proportion in the nature of the lesion and in its location was remarked. Hence, the plan of treatment I have recommended, was adopted, and has been carried out during a period of over twenty-five years.
