Book excerpt: The Balanced Budget Act of 1997 authorized a three-year test--called Medicare subvention--that allows military retirees, their dependents, and survivors to enroll in a new Department of Defense (DOD) health maintenance organization (HMO). The goal is to offer accessible, quality care while keeping costs down for DOD and Medicare. Care for these beneficiaries at military treatment facilities is now provided on a space-available basis that lacks the continuity so important to many older retirees. Under the Medicare subvention demonstration, Medicare will pay DOD for health care provided to retirees at six sites. DOD will provide enrollees with the full range of Medicare-covered services as well as some additional ones. In principle, beneficiaries, DOD, and Medicare could all gain under subvention. GAO is required to report annually on the demonstration's effect on Medicare costs. The demonstration was not fully implemented at all sites until January 1999, so there is not yet enough evidence to assess subvention's cost to Medicare. Consequently, this report focuses on the ability of DOD's data systems to (1) determine DOD's historical "level of effort" and Medicare payments and (2) manage the demonstration and assess its costs effects.

Book excerpt: The Centers for Medicare and Medicaid Services (CMS, formerly the Health Care Financing Administration) and the Department of Defense (DoD) have been testing the feasibility of making Medicare-covered health care services available to Medicare-eligible DoD beneficiaries through the TRICARE program (the managed care program of the Military Health Service) and military treatment facilities (MTFs). The vehicle used was the Medicare-DoD Subvention Demonstration, which was established by the Balanced Budget Act of 1997 (BBA). The goal of the demonstration was to implement cost-effective alternatives for care for this dually eligible population while ensuring budget neutrality, that is, neither CMS's nor DoD's total costs increase. The Secretaries of the Department of Health and Human Services and of the Department of Defense executed a Memorandum of Agreement that specified how the subvention demonstration was to be designed and operated. The Memorandum provided for an independent evaluation of the demonstration, which RAND conducted. This report describes the final results of the RAND evaluation.

DOD's TRICARE health plans covered military retirees to age 65. 1 However, once eligible for Medicare, retirees were no longer eligible for TRICARE and could not enroll in DOD s managed care plan, TRICARE Prime. As nonenrollees, Medicare-eligibles had access to care at military treatment facilities (MTF) only to the extent that space not utilized by TRICARE s enrolled population was available. By law, DOD could not receive payments from Medicare and was not responsible for providing the full range of benefits to Medicare-eligibles. Recently, the situation has changed. In 1997, Congress authorized the DOD Medicare subvention demonstration for a 3-year period. 2 Under this demonstration, DOD formed Medicare managed care organizations that enrolled and served Medicare-eligible military retirees at six sites. The DOD Medicare plan, which was called TRICARE Senior Prime, combined TRICARE with Medicare benefits and requirements. The demonstration also authorized DOD to receive payment from Medicare if certain conditions were met. Senior Prime gave enrollees the same priority for military care as younger retirees enrolled in TRICARE Prime, with minimal out-of-pocket costs. With TRICARE Prime as its foundation, Senior Prime illustrated issues that arose in bringing older retirees into DOD managed care. A major change in health care arrangements for Medicare-eligible retirees from the uniformed services will take place October 1, 2001. Under provisions of the Floyd D. Spence National Defense Authorization Act for Fiscal Year 2001, 3 these retirees will become eligible for TRICARE and will be able to use their Medicare benefit within TRICARE. The new program is commonly termed TRICARE For Life. Experience under Senior Prime illustrated some issues that TRICARE might face.

Book excerpt: The Medicare-Department of Defense (DoD) Subvention Demonstration tested Senior Prime as a managed care approach for enhancing access to affordable health care for Medicare-eligible DoD beneficiaries. While Senior Prime achieved solid beneficiary participation and satisfaction, it also raised a difficult set of challenges involved in applying managed care to the DoD health care system. These challenges included financial issues such as establishing equitable capitation rates and an appropriate level-of-effort baseline, as well as management issues such as effective care management and administrative processes for health plan sites. The basic structures of TRICARE and the DoD health system, including separate management jurisdictions and hierarchical budgeting methods, contribute to the challenges by creating incentives that discourage delivery of cost-effective care. Military treatment facilities need to be motivated not only to provide excellent care but also to manage appropriateness of care and related costs. Although DoD has decided to discontinue the Senior Prime model, many of the lessons learned from this demonstration are applicable to any managed care program that DoD may contemplate in the future.
