As of early May 2020, Covid-19 had claimed the lives of more than 28,000 nursing home residents and staff in the United States. But U.S. nursing homes were unstable even before Covid-19 hit. The tragedy unfolding in nursing homes is the result of decades of neglect of long-term care policy. Beyond the pandemic, we will have to transform the way we pay for and provide long-term care. First, Medicaid programs need to invest considerably more in care in all settings, including home-based settings as Medicaid shifts a larger share of care into them. Second, because caregiving at home is not feasible for many care recipients and families, we also need safe, affordable residential options. Better residential options can help ensure that the tragedy currently unfolding in nursing homes never happens again. Finally, we believe that the United States needs to reconsider our piecemeal approach to paying for long-term care. Existing programs, such as Medicare and Medicaid, would have to fundamentally change the way they pay for long-term care to meet the needs of our aging population. We can look to other countries for more coherent financing systems. More funding alone is not the answer. Nor is more regulation a sufficient response. Rather, we need a combination of funding, regulation, and a new strategy that fully supports a range of institutional and noninstitutional care.

No group has been impacted as heavily as the residents of for-profit nursing homes. This first-hand account by a healthcare worker details the devastation caused by the virus once introduced into a home for the most vulnerable. What began with a few residents testing positive in the hospital quickly progressed to others in the facility developing symptoms with dozens dying and many others left with long-term health effects. There were many opportunities for management to intervene and minimize the spread. Instead, the virus was allowed to spread unchecked among residents and staff. Almost as disturbing as the illness itself is the skill with which management was able to convince the majority of the staff and public that their response was exemplary given the circumstances. The decisions made behind closed doors rarely put the wellbeing of the residents first. Lack of accountability has been a longstanding problem in for-profit nursing homes. While the residents suffer and families are left in the dark, the nursing home industry is lobbying for less liability and less regulation. The employees of this industry are overworked, understaffed and forced to choose between providing patient care, protecting themselves and defending the actions of their corporations. Without more oversight, these companies will continue to get rich by taking advantage of those who are uninformed or left with no other options.

The findings are based on 13,069 US nursing homes, and the results are mostly consistent with most recent studies around this topic. Thus, this study provides not only additional evidence for previously studied factors based on a larger population of nursing homes with a holistic approach but also complements these with features not yet examined, such as most importantly the competitive environment of a nursing home. The findings show evidence of a relationship between COVID-19 infections and fatalities and (1) the size of a nursing home, (2) a facility's age, (3) whether a nursing home is for-profit, (4) whether a nursing home is urban or rural, (5) the number of federal deficiencies, (6) the total amount of fines, (7) the concentration of residents with Medicaid, (8) the share of residents from a racial or ethnic minority, (9) the excess of beds in the respective county of a nursing home, (10) the number of infections per 100,000 people in a county, and (11) the number of deaths per 100,000 people in a county, (12) the occupancy rate, (13) the overall CMS facility rating, (14) the total reported RN staffing levels, (15) the total reported nurse staffing levels and (16) the Herfindahl Index.
