Book excerpt: Excerpt from The Chicago Journal of Nervous and Mental Disease, 1874, Vol. 1 As you have been already informed, the nervous system i divided into two parts, the cerebro-spinal and the sympathetic. The first consists of the brain and spinal cord, and the intervening parts, viz.: the medulla oblongata and pons varolii, and also the cerebral and spinal nerves. The second comprises the chain of ganglia, on either side of the spinal column, connected together by intervening cords, and giving off branches, on the one hand, to the cerebro-spinal nervous system, and, on the other hand, large numbers of branches outwardly, which weave plexuses that accompany the arteries to their final distribution, especially those which supply the viscera and glandular organs. There are several points, to which I now will direct your attention: 1. Its histological elements, and their distribution. 2. The relations which exist between this and the cerebrospinal nervous system. 3. The seat of vaso-motor centres in the brain and cord. I. Histological Elements, and their Distribution, The three elements of importance are: the nerve cells, nerve fibres, and the connective tissue. The nerve cells of the sympathetic, when compared, as a whole, with those of the cerebro-spinal nervous system, are smaller, and, as a rule, perhaps, are found with fewer poles, or "caudate processes." They are often, to say the least, bipolar. They are simpler than those of the brain and spinal cord. They are found chiefly in the ganglia of the sympathetic, to the formation of which they largely contribute. But they are found elsewhere. Small ganglia are found in organs like the uterus and heart, especially the latter. Concerning the ganglia found in the substance of the heart, I will speak to you at another time. Small ganglia, or even single nerve cells, have been found in many glandular organs, on the walls of the muscular vessels, and beneath the mucous membrane of the alimentary canal, in which latter part a very fine rete, or network, of cells exists. Doubtless, small ganglia exist in many other parts of the body, so that various organs may have, to a certain extent, a self-contained nervous supply, in case of accident to the more central and remoter parts of the nervous system. The table also includes two cases of general paresis unexpectedly found post mortem in the residue of 186 cases of this series (247 less 61 paretic). Seventy per cent. Of the cases in which general paresis was certain, probable, or even prominently considered, proved histo pathologically cases of general paresis. Less than I per cent. Of a series of 186 cases in which the diagnosis of general paresis was not considered, proved to be instances of general paresis. It is clear that the errors in diagnosis found in the clinically certain group are of the first importance in our diagnostic study. 85 per cent of these cases obtained the correct diagnosis at staff meeting. What is the cause of incorrect diagnosis in the remain ing 15 per cent? There were six of such cases, and it seems worth while to present abstracts of the clinical histories and autopsy protocols in these cases. The first wrongly diagnosed case was that of E. C., D. I. H. 11987, Path. Lab. 893, and has been amply described by Prof. A. M. Barrett as a case of severe progressive arteriosclerotic brain atrophy with tabes-like complications in his Study of Mental Diseases Associated with Cerebral Arteriosclerosis, Am. Four. Insanity, LXII, I, 1905, pp. 48 - 50. The confusing feature from the standpoint of diagnosis was the tabes-like picture; and histo pathologically there proved to be, in addition to bilateral pyra midal tract disease, a lumbosacral tabes or tabetiform disease. Both pupils fail to react to light. There is no paralysis of the external ocular muscles. The patient complains of pain in the right chest. There are present bronchial breathing and moist rales. Heart sounds are dull and distant. The pulse is slow and weak, though regular. Examined October 18, 1892. The left knee-jerk as before is exaggerated. There is present a faint left ankle clonus. The right knee jerk continues to be normal. A Babinski reflex is present upon both sides, but not quite as marked as at the pre vious examination. The right arm is flaccid, the right leg is flaccid. The left arm is less rigid than upon admission, though decided resistence to movements is still present at the elbow. The wrist and fingers are less rigid than at the first examination. Otherwise there is no decided change. The patient is evidently growing weaker. There is a slight rise of temperature. Pulse is rapid, while the breathing is stertorous. The patient is now quite stuporous. The examination of the urine reveals the presence of some hyalin and a few granular casts with a small quantity of albumin.
