Book excerpt: This dissertation examined the association between participation in school meal programs and weight status among 12 to 15 year olds using data from National Health and Nutrition Examination Survey (1999--2006). Multinomial logistic regression analysis was used to examine the data concerning 12 to 15 year old Black, White, and Hispanic students who attended schools that participated in the School Breakfast Program and the National School Lunch Program, while controlling for race, gender, age, physical activity, poverty, and the survey period and treating race or a combination of race and poverty income ratio as effect modifiers. Initial exploration of potential non-linear association revealed that: the school breakfast-weight status association could be modeled as linear type; and school lunch-weight status association could be modeled as a reverse-V shape. Race was found to be an effect modifier for the school breakfast-weight status association and not for school lunch-weight association. Among Blacks, breakfast participation showed non-significant positive association with at risk of overweight (OR=1.06 for each increase in weekly breakfast participation, p = 0.17) and with overweight (OR= 1.02, p = 0.58). Among Whites, breakfast participation was positively associated with overweight (OR=1.12, p = 0.06) and negatively associated with at risk of overweight (OR=0.88, p = 0.10). There was a statistically significant association between school lunch participation and overweight (p = 0.01). Shift of lunch participation from zero to 1 is associated with increased odds of being overweight. (OR=2.09, p = 0.01), and further increase in lunch participation beyond 1 per week was associated with decreased odds of being overweight (OR for per each weekly lunch participation = 0.85, p = 0.01). The results raise some concerns over whether school breakfast is serving the intended purpose. Frequent breakfast participants among Whites were found to have high probability of being overweight and can be considered as a target for intervention. School lunch participants and non-participants showed marked difference in probability of being overweight. Racial differences in the association between meal participation and weight status and the underlying reasons thereof should be explored for further study, with consideration given to all foods consumed throughout the day. -- Abstract.

Statements were made by several senators, the president of the American School Food Service Association (Connecticut); a school food service program director (Florida); the director of nutrition and education for the American School Food Service Association (Virginia); the director of the Children's Nutrition Research Center (Texas); the vice president of Food Operations at Disneyland Resort and Concept Development (California); and an assistant professor of clinical dietetics and nutrition, University of Pittsburgh (Pennsylvania). Witnesses maintained that the Nutrition Education and Training Program needs funding to maintain its current service level. It is necessary to modify the School Lunch Program to encourage student participation, make it easier for schools for qualify, and to reduce the administrative burden. Testimony also indicated that breakfast programs are vital to student nutrition, academic performance, and school attendance. School food service has become more efficient and more consumer-oriented, has expanded to supply meals to new audiences, and has increased the flexibility of service delivery. However, it is necessary to integrate food service within the educational day to enhance student nutrition knowledge and practice. Offering food choices may reduce food waste. The solution to major childhood nutritional problems is to apply current information to improve children's diets and to support nutrition research. Book excerpt: In light of the recent rise in childhood obesity, the School Breakfast Program (SBP) and National School Lunch Program (NSLP) have received renewed attention. Using panel data on over 13,500 primary school students, we assess the relationship between SBP and NSLP participation and (relatively) long-run measures of child weight. After documenting a positive association between SBP participation and child weight, and no association between NSLP participation and child weight, we present evidence indicating positive selection into the SBP. Allowing for even modest positive selection is sufficient to alter the results, indicating that the SBP is a valuable tool in the current battle against childhood obesity, whereas the NSLP exacerbates the current epidemic.

Book excerpt: The National School Lunch and School Breakfast Programs, administered by the Food and Nutrition Service (FNS) of the U.S. Department of Agriculture (USDA), are key components of the nation's food security safety net, providing free or low-cost meals to millions of schoolchildren each day. To qualify their children each year for free or reduced-price meals, many families must submit applications that school officials distribute and review. To reduce this burden on families and schools and to encourage more children to partake of nutritious meals, USDA regulations allow school districts to operate their meals programs under special provisions that eliminate the application process and other administrative procedures in exchange for providing free meals to all students enrolled in one or more school in a district. FNS asked the National Academies' Committee on National Statistics and Food and Nutrition Board to convene a panel of experts to investigate the technical and operational feasibility of using data from the continuous American Community Survey (ACS) to estimate students eligible for free and reduced-price meals for schools and school districts. The ACS eligibility estimates would be used to develop "claiming percentages" that, if sufficiently accurate, would determine the USDA reimbursements to districts for schools that provided free meals to all students under a new special provision that eliminated the ongoing base-year requirements of current provisions. Using American Community Survey Data to Expand Access to the School Meals Program was conducted in two phases. It first issued an interim report (National Research Council, 2010), describing its planned approach for assessing the utility of ACS-based estimates for a special provision to expand access to free school meals. This report is the final phase which presents the panel's findings and recommendations.

Book excerpt: The National School Breakfast Program feeds 10 million children each day, and the National School Lunch Program feeds more than 30 million students. Yet the national nutrition standards and meal requirements for these meals were created more than a decade ago, making them out of step with recent guidance about children's diets. With so many children receiving as much as 50 percent of their daily caloric intake from school meals, it is vital for schools to provide nutritious food alongside the best possible education for the success of their students. At the request of U.S. Department of Agriculture (USDA), the Institute of Medicine assembled a committee to recommend updates and revisions to the school lunch and breakfast programs. The first part of the committee's work is reflected in the December 2008 IOM report Nutrition Standards and Meal Requirements for National School Lunch and Breakfast Programs: Phase I. Proposed Approach for Recommending Revisions. Phase II of the report is expected in Fall 2009. This first report provides information about the committee's approach as it reviews the school lunch and breakfast programs. In the report's second part, the committee will share its findings and recommendations to bring these meals more in line with today's dietary guidelines. The committee welcomes public comments about its intended approach. An open forum will be held January 28, 2009 in Washington, DC to receive input from the public. Book excerpt: There has been significant media attention on the issue of childhood obesity, leading policymakers to reform the National School Lunch Program (NSLP) to include stricter nutritional requirements. We use data on school lunch menus to document improvements in the nutritional quality of school meals between 1991 and 2010. We then evaluate how this change in nutritional content maps into obesity outcomes, using panel data on a nationally representative cohort of children, tracking them from kindergarten entry in fall 2010 through the end of fifth grade in spring 2016. We find little evidence that participation in the school lunch program leads to weight gain, as measured by changes in obesity, overweight, and BMI. These results suggest that improvements in the nutritional content of school lunches have been largely successful in reversing the previously negative relationship between school lunches and childhood obesity. Unrelated to school lunch participation, we find a strong relationship between mother's obesity status and both the level and growth of children's obesity, especially for girls and among high-SES families.

Book excerpt: Objective: The purposes of this study were two-fold: 1) to assess the school breakfast environment at four rural Appalachian schools for the contribution of foods to calories, fat, and fiber; and 2) to assess the dietary intake of students in these schools in relation to where breakfast was consumed (home, school, or both places) and by student weight status. Setting: Four rural Appalachian schools with fourth and fifth grade students in East Tennessee. Subjects: 255 fourth grade children completed a 24-hour dietary recall with a trained NDS-R interviewer and were weighed and measured by the Coordinated School Health Program. Design: Assessment of baseline data from an intervention study targeting 4th and 5th grade students in one rural East Tennessee county, Youth Can!, was used. School food service managers submitted school menus and production sheets for 18 days, and vendor bid sheets for analysis of the school breakfast environment. NDS-R software was used to analyze each breakfast food item for calories, fat, and fiber content per serving and production sheets were used to determine amounts of each breakfast food item served. Dietary recalls for days when school breakfast could be consumed were analyzed for energy and target nutrients using NDS-R software. Weight status was calculated as at risk of or overweight and not at risk of overweight based on BMI percentile for age. Statistics: Descriptive statistics were used to describe the school breakfast environment in terms of calories, fat and saturated fat (grams, percent calories) and fiber (grams) from foods sold on a per person basis. Food items also were grouped by the five meal components of the School Breakfast Program and ranked according to the total items served. Relationships between dietary intake and breakfast location and child weight status were evaluated using analysis of variance. Relationships between breakfast location and child weight were examined using chi-square tests. Results: On average in these school environments fat provided slightly less than half the calories (43%); 15% of calories were from saturated fat. The top ranked foods for servings sold for each meal component were biscuits, sausage, 2% milk, orange juice, and gravy. Children consuming breakfast at home and school had significantly higher percent breakfast contribution to the entire day for energy and calcium compared to children who only ate breakfast at home or school. While children who ate breakfast at home had significantly lower percent breakfast contributions to the entire day for percent calories from fat, protein compared to children who ate at school. Children who ate breakfast only at school had lower percent breakfast contribution to the entire day for iron and vitamin A compared to children who ate breakfast only at home. Breakfast consumption regardless of location had no impact on child weight status. Conclusion: The high fat content school breakfast environment reinforces the importance of healthy school food policies and technical support and resources for food service programs to provide low-fat meal options. However, children are not consuming all the breakfast items being served at school. Further research is needed to determine the impact physical activity and socio-economic status have on weight.

Book excerpt: This report investigates three important aspects of National School Lunch Program (NSLP) and School Breakfast Program (SBP) participation using recently collected data on a large, nationally representative sample of students certified for free and reduced-price meals during the 2005-2006 school year. First, we examine the factors that influence the participation decisions of children from low-income households. In this analysis, school type and student attitudes toward school meals emerged as factors with particularly strong relationships for both school lunch and school breakfast participation. Second, we examine the relationship between school meal certification status and participation using meal certification error as a natural experiment. The most policy-relevant finding from this analysis is evidence that eliminating the reduced-price certification category would increase lunch participation among students eligible for reduced-price meals under the current program rules, but that it would not increase breakfast participation. Finally, we explore the extent to which parent reports of student NSLP/SBP meal participation accurately represent actual school meal participation as determined from school administrative records. This analysis suggests that using parent reports on a short period of time leads to overstatements of actual school meal participation over longer periods, and could cause researchers to incorrectly identify the relationship between participation and other factors.
