In doing so, it offers a step-by-step guide through the key questions needed to model an evacuation and its impacts to the evacuation route system as well as evacuation management strategies for influencing demand and expanding capacity. The authors also demonstrate how to simulate the resulting traffic and evacuation management strategies that can be used to facilitate evacuee movement and reduce unnecessary demand. Case studies, which identify key points to analyze in an evacuation plan, discuss evacuation termination and re-entry, and highlight challenges that someone developing an evacuation plan or model should expect, are also included. Beyond this, public health today is challenged in a way that it has never been before. Starting with the 9/11 terrorist attacks, public health officers have had to spend significant amounts of time addressing the threat of terrorism to human health. Hurricane Katrina was an unprecedented disaster for the United States. During the first weeks, the enormity of the event and the sheer response needs for public health became apparent. The tragic loss of human life overshadowed the ongoing social and economic disruption in a region that was already economically depressed. Hurricane Katrina reemphasized to the public and to policy makers the importance of addressing long-term needs after a disaster. On October 20, 2005, the Institute of Medicine's Roundtable on Environmental Health Sciences, Research, and Medicine held a workshop which convened members of the scientific community to highlight the status of the recovery effort, consider the ongoing challenges in the midst of a disaster, and facilitate scientific dialogue about the impacts of Hurricane Katrina on people's health. Environmental Public Health Impacts of Disasters: Hurricane Katrina is the summary of this workshop. This report will inform the public health, first responder, and scientific communities on how the affected community can be helped in both the midterm and the near future. In addition, the report can provide guidance on how to use the information gathered about environmental health during a disaster to prepare for future events.

Book excerpt: Background: the lower southeast Florida region, comprised of Monroe, Dade, Broward and Palm Beach Counties, has been identified by the National Oceanic and Atmospheric Administration as one of the most hurricane vulnerable areas of the United States. The U.S. Army Corps of Engineers, Jacksonville District, recognizing the need to identify and measure time components critical to the hurricane evacuation process, provided funding through its Flood Plain Management Services Program and leadership to accomplish this study. Funding assistance was also provided by the Federal Emergency Management Agency (FEMA). Detailed technical study results are provided through this Technical Data Report. Implementation Reports will be developed subsequently by county civil defense officials with the assistance of the Jacksonville District, Corps of Engineers to provide a decision making guide for local county officials. Public information materials will then be developed by the South Florida and Treasure Coast Regional Planning Councils. Major technical work tasks were performed by Post, Buckley, Schuh & Jernigan, Inc. under contract to and under the guidance of, the Jacksonville District, Corps of Engineers.

Book excerpt: In September 2005, the threat of Hurricane Rita to the Houston-Galveston region resulted in the evacuation of more than one million persons from the Texas coastal and urban areas. This concentration of evacuees overloaded all routes leaving the Houston area, resulting in traffic queues on freeways as long as 36-hours. While traffic and emergency management personnel had access to real-time traffic information in the urban areas, no such information was available in rural areas where the roadways had restricted capacity. There was clearly a need to expand video and traffic monitoring capabilities beyond the urban areas to allow for better traffic management in response to evacuations as well as normal traffic operations. HURREVAC is a restricted-use Internet based computer program used by government emergency managers to track hurricanes and assist in evacuation-related decision making. The program includes an ETIS (Environment Transport Integrated planning System) module included which allows for inclusion and access to real-time traffic information by emergency managers. This report documents attempts to complete the integration of the module with traffic data for Texas coastal regions. It also provides for general equipment and installation guidelines for deployment of video and sensor detection stations along hurricane evacuation routes. In addition, a list of recommended additional deployments of such monitoring equipment to allow for improved monitoring and managing of traffic during evacuation events in Texas.

Book excerpt: US-Mexico border region area has unique social, demographic and policy forces at work that shape the health of its residents as well as serves as a microcosm of migration health challenges facing an increasingly mobile and globalized world. This region reflects the largest migratory flow between any two nations in the world. Data from the Pew Research Center shows over the last 25 years there has never been lower than 140,000 annual immigrants from Mexico to the United States (with peaks over 700,000). This migratory route is extremely hazardous due to natural (e.g., arid and hot desert regions) and human made barriers as well as border enforcement practices tied to socio-political and geopolitical pressures. Also, reflecting the national interdependency of public health and human services needs, during the most recent five year period surveyed the migratory flow between the US and Mexico has equaled that of the flow of Mexico to the US--both around 1.4 million persons. Of particular public health concern, within the US-Mexico region of both nations there is among the highest disparities in income, education, infrastructure and access to health care--factors within the World Health Organization’s conceptualization of the Social Determinants of Health, and among the highest rates of chronic disease. For instance obesity and diabetes rates in this region are among the highest of those monitored in the world, with adult population estimates of the former over 40% and estimates in some population sub-groups for the latter over 20%. The publications reflected in this Research Topic, all reviewed from experts in the field, addressed many of the public health issues in the US Mexico Border Health Commission’s Healthy Border 2020 objectives. Those objectives-- broad public health goals used to guide a diverse range of government, research and community-based stakeholders--include Non Communicable Diseases (including adult and childhood obesity-related ones; cancer), Infectious Diseases (e.g., tuberculosis; HIV; emerging diseases--particularly mosquito borne illnesses), Maternal and Child Health, Mental Health Disorders, and Motor Vehicle Accidents. Other relevant public health issues affecting this region, for example environmental health, binational health services coordination (e.g., immunization), the impact of migration throughout the Americas and globally in this region, health issues related to the physical climate, access to quality health care, discrimination/mistreatment and well-being, acculturative/immigration stress, violence, substance use/abuse, oral health, respiratory disease, and well-being from a social determinants of health framework, are critical areas addressed in these publications or for future research. Each of these Research Topic publications presented applied solutions (e.g., new programs, technology or infrastructure) and/or public health policy recommendations relevant to each public health challenge addressed.
