Book excerpt: Introduction : la nutrition de la femme enceinte est une préoccupation ancienne dans nos sociétés car elle est un facteur important du bon développement fœtal. En France, la complémentation des femmes enceintes est de plus en plus fréquente. L'objectif principal de ce travail était d'évaluer la prévalence de la consommation de compléments alimentaire chez la femme enceinte. L'objectif secondaire était d'évaluer la prévalence de la consommation de compléments alimentaires par rapport aux facteurs associés tels que le score EPICES, l'indice de masse corporelle et le régime alimentaire. Patientes et Méthodes : une étude observationnelle descriptive auprès de 250 femmes ayant accouché au sein d'un Centre Hospitalier de type III a été réalisée à l'aide de questionnaires auto-administrés. Les parturientes sélectionnées étaient hospitalisées dans les services de suites de couches à J2 et J3 du post-partum et avaient donné leur accord au préalable. Les critères d'exclusion étaient : les femmes ne comprenant ou ne parlant pas le français, une interruption médicale de grossesse, un nouveau-né devant être prise en charge dans le cadre de soins de réanimation ou néonataux, une mort fœtale in utéro. Résultats : la prévalence de la consommation de compléments alimentaires chez la femme enceinte était de 49,60 %. Il s'est avéré que les femmes en situation de précarité consommaient moins de compléments alimentaires, l'IMC n'a démontré aucun effet sur la prise de complémentations et pour finir le manque d'exhaustivité à propos des régimes alimentaires n'a pas permis la réflexion. En outre, la consultation préconceptionnelle est encore trop peu réalisée. Discussion et Conclusion : les compléments alimentaires restent très prescrits alors qu'aucun intérêt n'a été démontré en leur faveur. Ces résultats montrent que les recommandations mériteraient une meilleure diffusion auprès des professionnels de santé ainsi que des femmes en âge de procréer afin d'être plus sensibilisés à l'importance de la nutrition et de la complémentation éclairée pendant la grossesse.

Book excerpt: Objectifs : Déterminer la prévalence de la consommation en compléments alimentaires (CA) chez les femmes enceintes dans leur première moitié de grossesse. Déterminer les caractéristiques des compléments consommés et évaluer une association entre les croyances sur les médicaments et la prise de CA pendant la grossesse. Méthodes : Étude observationnelle, prospective et monocentrique au CHU Grenoble-Alpes, incluant les femmes enceintes entre 20 et 25 semaines d'aménorrhée. Les croyances ont été évaluées grâce à un outil standardisé : le Belief about Medicines Questionnaire (BMQ). Résultats : 41 femmes ont été incluses dont 53,66 % prenaient des compléments alimentaires, majoritairement orientées par un professionnel de santé, achetés en pharmacie, dans le cadre d'une prise préventive. Nous n'avons pas observé d'association significative entre les croyances sur les médicaments et la consommation de CA. Discussion et conclusion : La prescription de CA par les professionnels de santé est plutôt répandue chez les femmes enceintes dans leur première moitié de grossesse et cela malgré l'absence de recommandation sur le sujet. La promotion d'une alimentation saine et équilibrée est probablement à promouvoir.

Book excerpt: Les carences pouvant être de plus en plus fréquente par notre alimentation transformée, les organismes de santé veillent à éviter ce phénomène par l'établissement de recommandations. Au cours de la grossesse, les prescriptions de supplémentation actuellement très fréquente, diffèrent selon les praticiens de telle sorte que ces prescriptions deviennent soit automatiques, soit inadaptées pour les femmes enceintes. Nous avons mené une enquête auprès des sages-femmes réalisant des consultations sur le Grand Est afin de faire un état des lieux des pratiques sur la supplémentation pendant la grossesse. Nous avons obtenu 103 réponses par l'utilisation d'un questionnaire en vue de faire une étude descriptive et observationnelle. Notre analyse a montré les limites des recommandations des autorités de santé et l'impact limité sur la pratique des professionnels. Concernant les micronutriments indispensables à supplémenter pendant la grossesse, il reste un travail d'information à effectuer auprès des professionnels puisque pour encore 20% des sages-femmes la prescription d'acide folique n'est pas indispensable. Les compléments alimentaires sont beaucoup trop utilisés, également des éléments comme le magnésium non indispensable alors qu'aucun intérêt n'a été démontré à leurs supplémentations. Les conseils alimentaires ne sont pas assez largement diffusés. De même la réalisation d'un interrogatoire sur les habitudes alimentaires des femmes est trop peu pratiquée : seulement 58% des sages-femmes interrogées. Pourtant il paraît indispensable pour pouvoir adapter la prise en charge nutritionnelle et pratiquer une prescription en adéquation avec les recommandations. Il existe déjà des directives mise en place comme le Programme National Nutrition Santé (PNNS) mais les recommandations en matière de supplémentation auprès des femmes enceintes doivent être réactualisées et être plus largement diffusées auprès des professionnels de santé. Le PNNS de 2019-2023 prévoit justement de promouvoir les nouvelles recommandations du PNNS, de les diffuser et de faciliter leur mise en œuvre.
