Differential growth creates form: external form through growth rates which vary from one part of the body to another and one tissue to another; and internal form through the series of time-entrained events which build up in each cell the special ized complexity of its particular function. We make no distinction, then, between growth and development, and if we have not included accounts of differentiation it is simply because we had to draw a quite arbitrary line somewhere. lt is only rather recently that those involved in pediatrics and child health have come to realize that growth is the basic science peculiar to their art. It is a science which uses and incorporates the traditional disciplines of anatomy, physiology, biophysics, biochemistry, and biology. It is indeed a part of biology, and the study of human growth is a part of the curriculum of the rejuvenated science of Human Biology. What growth is not is a series of chärts of height and weight. Growth standards are useful and necessary, and their construction is by no means void of intellectual challenge. They are a basic instrument in pediatric epidemiology. But they do not appear in this book, any more than clinical accounts of growth disorders. This appears to be the first large handbook-in three volumes-devoted to Human Growth.

The digital anatomy correlation used in this work is the natural continuation of efforts to apply the University of Colorado Visible Human data set to gastroenterology. The Visible Human data set was created by Dr. Vic Spitzer and colleagues at the University of Colorado and is currently housed at the universitys Center for Human Simulation. The data set consists of high resolution transaxial digital images captured as cadavers were abraded away at 1 mm or less depths. These images are compiled into blocks of data and each structure is identified. This information can be used to pull out and manipulate 3-D structures as well as allowing one to review planar anatomy in any orientation. Using the Visible Human dataset, one should be able to find a normal anatomy correlate to any image found during a EUS examination. However, as important as normal anatomy is, it is the abnormal features which are the crux of an EUS examination. Endosonographers are asked to define lumps, bumps, cysts to find correlates for symptoms and abnormal laboratory findings. Accuracy requires a tremendous amount of skill and experience. To help in this task, we have assembled chapters from a world-wide group of expert endosonographers. These authors have shared their insight and images to help the readers of this work better see and understand some of the complexities uncovered during a EUS evaluation. Sir Isaac Newton (1675) Although congenital displacement of the hip has always been recognised as the commonest pre-natal deformity of the musculoskeletal system found in otherwise normal children, it is surprising to find that much of our present-day understanding concerning its origin and nature has been discovered only in the past 150 years and its successful management eventu ally emerged during the last 60 years, almost within the professional life time of our more senior colleagues. Whereas Hippocrates (460-370 B. C. ) appeared to recognise patients that had experienced dislocation of their hips in utero and identified them from acquired forms of displacement, according to Severin (1941) it was another 2000 years before Palleta (1820) first recorded a careful description of the deformity based on his observations at an autopsy performed on an II-day-old boy with bilateral dislocations; he concluded that the findings were not caused by injury at birth, but dated from a pre-natal stage. Soon after, Baron Dupuytren (1847) gave an accurate description of congenital dislocation in his contribution entitled "Memoire sur un deplacement original ou congenital de la tete des femurs". He noted the absence of abscesses and fistulae as seen in painful and cruel pathological dislocations, which led him to believe that these congenital displacements were not likely to be the result of foetal disease as the affected babies appeared quite healthy at birth.
