Available in PDF, EPUB and Kindle. Book excerpt: Introduction : La fausse couche spontanée (FCS) est la complication prénatale la plus fréquente avec 200 000 FCS par an en France. Les complications psychologiques qui en découlent ont été largement documentées mais peu d’études se sont interéssées aux répercussions psychologiques sur le vécu ultérieur de la femme et notamment le vécu d’une nouvelle grossesse. Objectifs : Evaluer l’impact d’un antécédent de FCS sur le niveau de stress des femmes enceintes au premier trimestre de grossesse. Méthode : Etude observationnelle, comparative de type « exposés/non-exposés » à l’antécédent de FCS. Entre juin et octobre 2021, diffusion d’un auto-questionnaire par mail ou sms auprès de femmes majeures, enceintes au premier trimestre de la grossesse et acceptant de participer à l’étude. Evaluation du niveau de stress par l’échelle « antenatal perceived stress inventory » (APSI). Résultats : 93 femmes ont été incluses (63 sans et 30 avec antécédent de FCS). L’antécédent de FCS n’était pas associé au score global de l’APSI mais était significativement associé au facteur 1 de l’APSI « risques médico-obstétricaux et la santé du fœtus » avec un niveau de stress pour ce facteur plus élevé chez les femmes avec un antécédent de FCS (3.00 ± 0.86 vs 2.34 ± 0.80 ; β=0.61 (0.25-0.96) ; p = 0.001). Plus spécifiquement, l’antécédent de FCS était significativement associé aux items concernant « la santé du bébé » (p = 0.048) et « l’échographie » (p =0.002). Conclusion : L’étude montre l’impact d’un antécédent de FCS sur le niveau de stress au premier trimestre de la grossesse concernant les risques médico-obstétricaux et la santé du fœtus témoignant de l’importance d’une prise en charge psychologique adaptée.
