Book excerpt: Este estudo apresenta uma análise numérica pelo método de elementos finitos da distribuição de tensões em dentes restaurados com coroas unitárias totalmente cerâmicas de diferentes tipos de terminações cervicais. Para a obtenção do modelo numérico bi-dimensional, três modelos da região de molares inferiores foram criados a partir de uma peça anatômica real e simularam a aplicação de coroas cerâmicas puras com diferentes terminações cervicais no segundo molar. O material selecionado para as restaurações foi uma cerâmica reforçada por dissilicato de lítio, enquanto queas terminações cervicais adotadas foram do tipo ombro reto, ombro arredondado e chanfro. Com o objetivo de realizar uma análise mais criteriosa das distribuições de tensões inerentes aos diferentes tipos de terminaçõescervicais, optou-se por criar o mesmo modelo em uma condição totalmente hígida, isto é, com ausência de qualquer tipo de restauração. A simulação numérica foi realizada pelo software ANSYS 7.0, sendo os modelos carregados com uma força estática de 400 N distribuída uniformemente na espessura dos dentes e subdividida em pontos de aplicação nas superfícies oclusais dos elementos dentais de acordo com a relação cúspide-crista. Os vetorescorrespondentes à aplicação das forças foram direcionados para região ânteroinferior da mandíbula com uma angulação de 15o. Os resultados mostraram que a restauração envolvendo a coroa cerâmica com terminação cervical em ombro reto concentrou mais tensões no dente molarrestaurado, seguida pela coroa cerâmica com uma terminação cervical do tipo ombro arredondado. Por sua vez, a coroa cerâmica com a terminação cervical em chanfro provocou uma distribuição de tensões mais homogênea no dente molar restaurado.

Book excerpt: Porcelain-fused to-metal (PFM) crowns have been considered the gold standard for the repair of damaged teeth. PFM crowns have good mechanical properties, satisfactory esthetic results, and an acceptable biological quality needed for periodontal health. However, PFM crowns have some limitations that may limit their use. The fabrication of PFM is a highly technique-sensitive procedure that consists of investing wax patterns and casting precious metal alloys. Furthermore, the esthetic of PFM crowns is limited by the metal framework and the layer of opaque porcelain needed for masking the underlying metal grayish shade. Recently the cost of precious metals has risen markedly making PFM relatively unattractive from an economic standpoint. Ceramic crowns have been tried in the last four decades to replace PFM crowns and overcome their esthetic limitations. However, the use of all-ceramic crowns has been challenged in practice by the uncertainty of their physical proprieties and their resistance to fracture and chipping. Policy makers require information on the relative benefits and costs associated with different types of crown materials in order to support reimbursement decisions. The objective of this review is to evaluate the clinical and cost-effectiveness of dental PFM and all-ceramic crowns.

Surface modification technologies for implants have been applied to titanium at the micro-level for about four decades. Now, implant surfaces are being topographically and chemically modified at both the micro- and nano-level. The modification techniques are altering other metals and ceramics, making these materials more biocompatible. Materials for abutments in dental implant systems appear to depend on implant–abutment connection structures. Biomechanical factors, such as friction and preload, influence the development of the abutment materials. Additionally, the surfaces of the abutment materials are important in the soft-tissue attachment, which is being actively investigated. As dental implants have to be functional in human bodies for a long time, numerous materials are being clinically tested as implant-supported restorations.
