Il a pour but de clarifier les principes chirurgicaux et d'aider à l'élaboration d'une stratégie des révisions des prothèses totales de genou. Simplification, c'est le maître mot qui doit animer le chirurgien, pour qu'il s'y retrouve : dans la diversité du matériel prothétique et la multitude des containers des ancillaires. L'équilibrage, c'est la clef de la compréhension et de la simplification. Derrière des noms propres (Burmester, Munchik,Neyret) se cachent deux idées simples: la conservation de l'interligne articulaire et la tension ligamentaire. La reprise simplifiée est régie par le théorème cinématique suivant : " Si l'on conserve le niveau de l'interligne articulaire, pour obtenir une articulation isométrique, il suffit d'avoir la même tension ligamentaire en flexion et en extension. " Retenons trois principes de base régissant la reprise d'une PTG par une prothèse contrainte postéro-stabilisée : les ligaments latéraux doivent être intacts, la détermination du niveau de l'interligne articulaire en préopératoire est fondamentale, la tension ligamentaire (longueur) en flexion et en extension doit être la même.

Book excerpt: Cette étude rétrospective sur une période d'un an a été réalisée afin de définir dans un premier temps le saignement médian propre au CHU de Nantes pour la pose de prothèse totale de hanche et de genou (PTH, PTG). Dans un second temps, la population à risque de transfusion homologue a été définie pour laquelle une stratégie d'épargne sanguine a été élaborée. Pour la pose de PTH et de PTG respectivement, le saignement médian était de 676 ml et de 847 ml, 72 et 71 des patients ont été transfusés en sang homologue et autologue, 44 et 54 en sang homologue seul. Les facteurs indépendants de transfusion homologue retrouvés étaient l'expérience du chirurgien, un hématocrite à la consultation d'anesthésie inférieur à 40 et pour les PTH, l'intégration à un programme de transfusion autologue différée (TAD). Grâce au protocole mis en place, l'EPO sera probablement plus utilisée avec une prescription plus raisonnée de la TAD.

Les prothèses totales du genou (PTG) engendrent un bénéfice fonctionnel significatif, démontré par la littérature scientifique. La prise en charge ambulatoire pour la chirurgie prothétique de genou avec une sortie à domicile à la 23ème heure post-opératoire a déjà été proposée dans la littérature. Le contrôle de la douleur est un des enjeux majeurs dans la prise encharge de la PTG en ambulatoire. Dans cette optique, le bénéfice apporté du bloc sciatique au cathéter fémoral a été récemment démontré. Le contrôle des pertes sanguines est également unparamètre fondamental à contrôler dans le cadre du remplacement prothétique de genou et de la chirurgie ambulatoire. L'objectif de cette étude est de valider la faisabilité du remplacement prothétique du genou en ambulatoire en étudiant la pertinence du bloc sciatique. Matériel et méthodes: Étude mono-centrique prospective randomisée contrôlée en simple aveugle. Les patients inclus dans l'étude entreront le matin dans l'unité de chirurgie programmée Hanche et Genou de l'IULS, au sein de l'hôpital Pasteur 2, et en sortiront l'après-midi pour être dirigés vers le centre de rééducation Atlantis. Trente-huit patients seront inclus et deux groupes de patients seront comparés, en simple aveugle : un groupe bénéficiera d'une analgésie par KT fémoral seul (groupe BS-) ; un groupe par KT fémoral et bloc sciatique (groupe BS+). Le critère de jugement principal est le taux de succès de l'ambulatoire lié au contrôle de la douleur (absence d'hospitalisation d'emblée ou de ré-hospitalisation en raison d'un contrôle insuffisant de la douleur postopératoire) jusqu'à J5 postopératoire. Les critères secondaires de jugement sont : la recherche d'évènements indésirables jusqu'à J5 postopératoire (intolérance à l'anémie, une transfusion post-opératoire, un événement thrombo-embolique documenté, une infection du site opératoire ou tout autre événements indésirables graves), l'évaluation de la satisfaction et du confort du patient par le score QOR-40. Résultats: Les résultats des premiers patients inclus dans l'étude nous confortent dans notre hypothèse de faisabilité des PTG en ambulatoire mais nous interpellent sur la pertinence du bloc sciatique puisque les patients BS- n'ont pas plus de douleur que les BS+. Compte tenu de ces résultats préliminaires, cette étude va être menée à son terme avec la même méthodologie. Discussion/Conclusion: Les niveaux de douleur des 8 premiers patients inclus dans l'étude ont tous été compatibles avec une sortie en ambulatoire. Les patients du groupe BS- n'ont pas présenté un niveau de douleur supérieur à celui du groupe BS+. Dans les 2 groupes il n'y a eu aucune hospitalisation secondaire. Les patients n'ont pas présenté d'événements indésirables. Leur score de confort et de satisfaction est très satisfaisant.

The number of PJIs is increasing worldwide and poses a real interdisciplinary challenge for everyone involved. For the patient concerned, it is necessary to promptly work out an adequate therapy solution to fight off the infection. Both the clinical experience of the surgeon and the proper diagnostic processes are prerequisite for the reliable detection and identification of an infection. The microbiologist is responsible for identifying the causative germs by screening the patient’s synovial fluid and tissue samples. Based on the findings and subsequent resistance testing, the infectious disease specialist can recommend the appropriate antibiotic therapy. Furthermore, the clinical pharmacist is consulted regularly throughout the therapy to discuss the risk of potential drug interactions. The surgeon will proceed with the revision surgery, following defined algorithms. Adequate radical debridement of infected and necrotic surrounding tissue is the most important step towards a successful cure of the infection. Accompanying the surgery, anti-infective agents are given systemically and locally. While systemic application of anti-infectives mainly reduces the number of haematogenic-spreading planktonic germs, local application immediately forms a colonization barrier and protects the implant from sessile biofilm formation. Concurrently, antibiotics are actively released from the implant, resulting in local germ reduction. Thus, local agents are embedded in the concept of surgical PJI treatment as a reliable adjuvant measure and they sustainably support the successful outcome. In one-stage procedures, local agents are released from specialized antibiotic-loaded bone cements, while in two- or multi-stage procedures, local agents are released from corresponding temporary spacers (interim prostheses). Even from an economic standpoint, the combination of systemic and local agent application is meaningful. Furthermore, there are some interesting trends towards the coating of metallic implants to protect against biofilm formation on the implant surface. On the basis of their personal experience, specialists from all over the world present, explain and discuss preventive approaches, appropriate diagnostic strategies for detection, reproducible effective surgical treatments as well as the economic impact of PJI. The reader can use this book as a solid platform for comparing their own approach to PJI treatment with the specialists’ recommendations.

In America, unlike anywhere else in the world, most people depend overwhelmingly on private health insurance and employee benefits. The astounding rise of this phenomenon from before World War II, however, has been largely overlooked. In this powerful history of the American reliance on employment-based benefits, Jennifer Klein examines the interwoven politics of social provision and labor relations from the 1910s to the 1960s. Through a narrative that connects the commercial life insurance industry, the politics of Social Security, organized labor's quest for economic security, and the evolution of modern health insurance, she shows how the firm-centered welfare system emerged. Moreover, the imperatives of industrial relations, Klein argues, shaped public and private social security. Looking closely at unions and communities, Klein uncovers the wide range of alternative, community-based health plans that had begun to germinate in the 1930s and 1940s but that eventually succumbed to commercial health insurance and pensions.
