Alexithymia, impulsivity and anger are only some of the possible factors causing aggression. Schizophrenic patients with violence history have over threshold STAXI-2 scores, showing how anger could predict aggression. Emotion disregulation can be mediator between alexithymia and aggression, but there is no evidence about impulsivity.ObjectiveThe purpose of this study (conducted in Barcellona PG forensic hospital before its disclosure) is to evaluate interactions between alexithymia, impulsivity, anger and their influence on specific crimes in a group of violent schizophrenic patients.Materials and Methods53 schizophrenic subjects with crimes Vs person/property were recruited and evaluated with TAS-20, BIS-11, STAXI-2. Exclusion criteria: intellectual disability. Statistical analysis: u03c72 test (non-continuous variables); t-test and post-hoc ANOVA (continuous variables).Results53 subjects, mean age: 40,2. 15 (28%) patients committed aggression/attempted homicide/sexual violence. 10 (19%) homicide/massacre. 11 (21%) persecutory acts/resisting arrest/extortion/gun license. 6 (11%) theft. 10 (19%) family maltreatment.24 (45%) u2265 59 (BIS-11+); 9 (15%) STAXI-2 over 75u00b0 percentile (STAXI-2+) (7 with ER/indice over 75u00b0 percentile)TAS-20 mean score: 55BIS-11 mean score: 58STAXI-2 ER/I mean score: 59Neither significant correlation (t-test) between TAS-20+ and BIS-11+, nor between TAS-20+ and STAXI-2+, nor between crimes and TAS-20+ , STAXI-2+ , BIS-11+.With BIS-11+ as mediator, TAS-20+ and STAXI-2+ association was statistically significant for the variables R/T (trait anger) (p=0,037), RT/T (trait temperament anger) (p=0,011), ER/IN (internalizing anger) (p=0,025)ConclusionsAlexithymia and impulsivity influence anger construct. With impulsivity as a mediator, alexithymia and trait/internalizing anger show their significant correlation.

Book excerpt: This book provides an overview of forensic psychiatry, focusing on the provision of care in Europe as well as the legal and ethical challenges posed by long-term stays in forensic settings. Forensic psychiatric services provide care and treatment for mentally disordered offenders (MDOs) in secure in-patient facilities as well as in the community. These services are high-cost/low-volume services; they pose significant restrictions on patients and hence raise considerable ethical challenges. There is no agreed-upon standard for length of stay (LoS) in secure settings and patients’ detainment periods vary considerably across countries and even within the same jurisdiction. Thus far, little research has been conducted to identify factors associated with length of stay; consequently, it remains unclear how services should be configured to meet the needs of this patient group. This volume fills some of those gaps. Furthermore, it presents new research on factors associated with length of stay, both patient-related and organisational. Various approaches to the provision of care for long-term patients in different countries are explored, including a few best practise examples in this specific area of psychiatry. The book also addresses the perspective of those working in forensic care by reviewing quality-of-life research and interviews with patients. The authors of this volume come from a range of professional backgrounds, ensuring a certain breadth and depth in the topic discussion, and even includes patients themselves as (co-)authors.

Book excerpt: Franco Basaglia (1924-1980) was an Italian psychiatrist and activist who proposed the dismantling of psychiatric hospitals and pioneered new ideas about mental health and its treatment. Basaglia was also one of the principal proponents of Italy's Law 180, which effectively closed down large mental hospitals in Italy. His ideas and his disciples have had a decisive influence in the move away from institutional care in many parts of the world, particularly in continental Europe and South America. However, Basaglia is strikingly absent from the literature in Germanic and Anglophone psychiatry. Most of the literature about Basaglia in the last 40 years has been published by his followers and supporters and has often been largely positive, with little exploration of differing responses or possible limitations of his model. Currently, there does appear to be a trend of increasing violence in hospital settings, including both civilly and forensically committed populations. In fact, physical aggression is the primary reason for admission to many hospitals. Given that violence is now often both a reason for admission and a barrier to discharge, there is a pressing need for violence to be re-conceptualized as a primary medical condition, not as the by-product of one. Furthermore, treatment settings need to be enhanced to address the new types of violence exhibited in inpatient environments and this modification needs to be geared toward balancing safety with treatment. This book focuses on violence from assessment, through underlying neurobiology, to treatment and other recommendations for practice.
