Book excerpt: Objective: The objective of this study is to evaluate the skeletal and dental stability of a Le Fort I maxillary downgraft after a minimum 4year follow up period. Methods: This retrospective cohort study evaluated 18 patients' post-surgical stability after Le Fort I maxillary downgraft performed by a Z-osteotomy. Pre-surgical (T1), post-surgical (T2), post-orthodontic (T3), and long-term follow-up (T4) cephalometric radiographs were digitized and analyzed with Dolphin Imagining via a custom analysis to evaluate linear and angular skeletal and dental relapse. Results -- The average Le Fort I maxillary down graft was 3.6mm. The average post-surgical orthodontic phase was 7.4 months and there was a significant amount of vertical relapse in the anterior maxilla in 20%-30% of the patients during this 7.4 month period. The average long-term follow up period was 5.64 years and during this period there were zero patients that had a significant amount of vertical maxillary movement. Conclusion - The Le Fort I maxillary Z-osteotomy with rigid internal fixation is a viable surgical technique to increase the stability of a maxillary downgraft. The stability of a Le Fort I maxillary Z-osteotomy to inferiorly position the maxilla after the removal of orthodontic appliances was shown to be highly stable. Even in patients where a significant amount of dental and skeletal relapse did occur, 100% of patients had positive overbite at time of debond and at time of long-term follow up.

Book excerpt: Cephalometric head films from a sample of 35 patients who had undergone a LeFort I maxillary osteotomy combined with a simultaneous mandibular advancement were evaluated to determine the postsurgical stability of the skeletal segments. Twenty-six of the patients had rigid fixation to stabilize the segments and nine patients had skeletal wire fixation. Twenty-seven cephalometric parameters were examined before surgery, immediately after surgery, and after a follow-up period averaging fifteen months. The results showed the maxilla to be stable for both types of fixation but the mandible was significantly more stable in the rigid fixation sample, especially in terms of the ability of rigid fixation to maintain rotational control between the proximal and distal segments. Measurements of cephalometric points representing resting muscle lengths indicated a propensity for the stretched muscles to relapse back to within 5 percent of their original lengths. Both groups showed a long term change in the hyoid bone position and head posture. Keywords: Dental surgery; Theses. (KT).

Book excerpt: Skeletal stability was examined in sixteen patients following combined maxillary and mandibular osteotomies using rigid internal fixation. The postoperative changes of all measured anatomic landmarks were generally less than 1.0mm for linear measurements, and less than 2.0 degrees for angular measurements. The removal of intermaxillary fixation (IMF) splints accounted for 85% to 95% of the counterclockwise rotation in the proximal and distal segments. Maxillary inferior repositioning and large mandibular advancements exhibited the greatest tendency for relapse; however, the changes were less than comparable procedures using non-rigid methods for stabilization. For a given category of surgical procedures, relapse was essentially unrelated to the magnitude of the surgical repositioning. Although the use of suspension wires, IMF, and transosseous wire fixation have traditionally provided satisfactory clinical results, the use of rigid internal fixation in combined double jaw procedures provides better stabilization of dentosseous segments when compared to non rigid fixation, and is particularly indicated in complex surgical procedures.
