Book excerpt: Excerpt from Southern Medical and Surgical Journal, 1852, Vol. 8 Much is to be anticipated from the further prosecution of these investigations, and already it is probable that they justify us in advancing one step in the pathology of the disease at the head of this article, and if the deductions are not erroneous, will lead to a fuller understanding of phlebitis in all its forms. The conclusions to which they would lead are, that phlegm asia dolens or phlebitis is caused by the introduction of diseased matter, usually pus, into the blood - that inflammation of a vein is not an essential part ofthe primary affection, which precedes constitutional symptoms, even when morbid matter has found its way into the circulation through a vein, and that when the veins are inflamed, it is an effect, and not the cause of the recep tion of diseased or foreign matter into them. I am informed that she had no spasmodic affection during or subsequently to her illness; but it was observed, when she began to run about again, that she was lame. This lameness gradually increasing, its cause was observed to be in the foot, whose distortion grew more and more obvious until I saw her, in August, 1854. The case then presented all the peculiarities of a well-marked Talipes varus, the weight of the body resting upon the external margin of the foot; one only being effected. Sometimes, in a case Sick for a week or ten days, we have not used more than one gentle purgative. Occasionally we have regretted the use of this - never its omission. The farinaceous arti cles of diet, which are allowed, together with the cold acidulous drinks, so grateful to the patient, are generally sufficiently laxative; and in protracted cases, these latter have often to be forbidden, owing to the great tendency to diarrhoea. Those addicted to the former will drink profusely for days or weeks, until the stomach rebels and rejects the potations, and they cannot take any more. They will then suffer greatly, mentally and physically, a few days, and gradual ly get over their spree, to resume it again after the lapse of weeks, months, or even years. This is the most inveterate, the most incurable form of intemperance. Indeed, I may say that I look upon such cases as utterly hopeless, for I have never known a single instance of permanent reformation, in those ad dicted to periodical intemperance. The victims of this form are more violent and uncontrolable, more disagreeable and danger ous to society, more subject to delirium tremens, and more liable to permanent insanity, than habitual inebriates.
