Book excerpt: Le chirurgien-dentiste dispose d'un arsenal thérapeutique permettant de remplacer l'organe dentaire perdu. L'implantologie est une solution confortable qui permet d'éviter le délabrement des dents bordant l'édentement ainsi que l'utilisation de prothèse amovible. Cependant, lors de la réhabilitation implanto-portée chez des sujets ayant des édentements maxillaires postérieurs, on peut être confronté à des difficultés chirurgicales liées à un os sous-sinusien atrophié. En effet, lors de la perte d'une dent, son tissu de soutien disparaîtra avec elle, jusqu'à laisser une fine membrane osseuse constituée du plancher sinusien et de l'os crestal. Cette épaisseur résiduelle étant insuffisante pour stabiliser l'implant, l'apport de la greffe sinusienne peut permettre de pallier à ce manque de volume osseux. L'évolution des techniques chirurgicales ainsi que des biomatériaux permettent d'avoir un large panel de choix de traitements. Il y aura en préambule, un rappel sur l'anatomie des sinus, ainsi que l'apport de l'imagerie médicale, puis les indications et contre-indications au sinus-lift seront déterminées. Les techniques de soulevé de sinus par voie latérale et par voie crestale seront ensuite décrites, et les cas dans lesquels il est indispensable de réaliser un comblement par biomatériaux seront définis. On identifiera les facteurs favorables et défavorables aux sinus-lift, ainsi que les cas où il est préférable de réaliser la chirurgie en un ou deux temps. Les complications per opératoires et postopératoires liées à ce type d'intervention seront citées. Dans un second temps, les différents types de biomatériaux seront abordés, ainsi que les conséquences pour le patient en l'absence de matériaux de comblement. Des études bibliographiques permettront de conclure s'il y a une différence significative sur la survie des implants entre les deux techniques de sinus-lift avec ou sans biomatériaux.

Les complications inhérentes à la chirurgie et la gestion de ces complications au cabinet dentaire mais aussi la prise en charge ORL seront abordées dans une dernière partie.
