Book excerpt: Government spending on health has grown as a percent of GDP over the last 40 years in industrialized countries. Widespread decentralization of healthcare systems has often accompanied this increase in spending. In this paper, we explore the effect of soft budget constraints on subnational health spending in a sample of OECD countries. We find countries where subnational governments rely primarily on central government financing and enjoy large borrowing autonomy have higher healthcare spending than those with more restrictions on subnational government borrowing.

Book excerpt: The reforms that have reshaped most public health care system have often been accompained by a process of devolution. However, this process has not always produced the desired effects and the existence of widespread soft budget constraint policies at local level is well documented. In this paper we argue that the soft budget constraint arise from a cooperative game between the authorities that are locally responsible for the provision and finance of health care. Our theoretical model is tested using data for Italian Regions for the period 2002-2006 and our hypothesis is verified. Although the model uses Italy as a benchmark, the results can be easily extended to any federal context where resources are unevenly distributed and preferences are asymmetric.

Book excerpt: Given an increasingly complex web of financial pressures on providers, studies have examined how the hospitals' overall financial health affect different aspects of hospital operation. In our study, we analyze this issue focusing on hospital access and quality by introducing an important aspect of the financial incentives, soft budget constraints (SBC), that takes into account both hospital's current and past financial health as well as their expected financial outlook (i.e., whether there is a sponsoring organization to bail them out). We develop a conceptual framework of SBC by considering the resultant incentives on cost control and quality improvement innovations; and examine the effect of SBC on the following aspects of access and quality: safety net service survival and AMI mortality rates. We find that hospitals with softer budget constraints are less likely to shut down safety net services. In addition, hospitals with softer budget constraints appear to have better mortality outcomes, suggesting that the reduced incentive to engage in cost control innovation as the result of SBC outweighs the dampening effect of quality improvement innovation.

In this paper we will concentrate on the related phenomenon of the soft budget constraint phenomenon. This framework can be traced back to that of a game where less efficient local authorities prefer to send their citizens to receive services outside their region instead of becoming more efficient. In order to improve the probability of being bailed out, the users are sent to other local authorities where there is excess capacity. The lack of coordination between local objectives and total welfare means that this policy is optimal at local level, but inefficient at Central Government level.

Available in PDF, EPUB and Kindle. Book excerpt: The hospital market is served by firms that are private for-profit, private not-for-profit, and government-owned and operated. I use a plausibly exogenous change in hospital financing that was intended to improve medical care for the poor to test three theories of organizational behavior. My results reveal that the critical difference between the three types of hospitals owes to the soft budget constraint of government-owned institutions. The decision-makers in private not-for-profit hospitals are just as responsive to financial incentives and are no more altruistic than their counterparts in profit-maximizing facilities. My final set of results suggests that the significant increase in public medical spending examined in this paper did not improve health outcomes for the indigent.

However, the latter must join and support the system of management as all its parts are equally important so, if one single element of the system, seemingly unimportant, is inefficient, the system collapses.
