Book excerpt: Ventilator-Associated Pneumonia (VAP) is an ongoing challenge that has significant morbidity and mortality rates. An abundance of bacteria in patients' mouths increase their susceptibility to bacterial infection. Unfortunately, current efforts to mediate VAP are not sufficient and result in an increased hospital stay and additional medical treatment (Hoshijima, 2013). Currently the national best practice is defined by the IHI Ventilator care bundle. The components of the bundle include 'Elevation of the Head of the Bed, Daily Sedation Vacations, and Assessment of Readiness to Extubate, , Peptic Ulcer Prophylaxis, Deep Venous Thrombosis Prophylaxis, and Daily Oral Care. Self reporting studies demonstrate oral care practices are significantly less than 100%. In addition, not all best practices have specified the use of chlorhexidine as part of the oral care (Turk, 2012). Moving to educate nurses and ensure compliance will drastically reduce the risk of unnecessary medical complications and increased medical costs. To justify the introduction of this additional practice, hospitals can easily implement an evaluation documentation and review plan that quantifies the savings. The following information explains the importance of adequate oral care, the challenges associated with it, and the potential benefits hospitals should expect to see after implementation.

Abstract: Ventilator-associated pneumonia (VAP) is a kind of frequently hospital-acquired infection that increases morbidity and mortality in patients in intensive care units (ICU), which would in turn increase length of ICU stay, cost of hospital stay, and duration of mechanical intubation. Pneumonia appeared within 48 hours before onset of infection in people whose respiration is mechanically supported through endotracheal tube or tracheostomy is ventilator-associated pneumonia. Combating VAP is a major challenge in ICU as almost all ICU patients require mechanical intubation. Development of evidence-based methods to reduce the incidence and prevalence of VAP becomes an important issue in ICU. The objectives of this dissertation are to conduct a thorough search of current evidence on the effectiveness of using different concentration, frequency of application, method of application and amount of chlorhexidine gluconate solution for oral care of adult intubated patients in reducing VAP incidence rate. MEDLINE (OvidSP), CINAHL PLUS (EBSCOhost), PubMed and British Nursing Index were used to conduct electronic search using keywords related to VAP. A total of 99 studies were identified and seven were selected according to inclusion criteria. The quality of the seven selected studies was tested using The Scottish Intercollegiate Guidelines Network (2008) tools for randomized controlled trials, and the evidence level coding from Scottish Intercollegiate Guidelines Network was used in grading of recommendations. Six studies were rated as high quality, which oral care using chlorhexidine gluconate solution had shown statistically significant VAP incidence rate reduction or VAP-related parameters improvement. Analysis on the implementation potential, transferability of findings, feasibility of implementation and cost-benefit ratio was conducted and the oral care guideline was beneficial to intubated patients. Implementation plan, communication plan and evaluation plan about oral care guideline application was formulated. The program designed to apply the new oral care guideline would last for one year, which includes communication with stakeholders, publication of the guideline, training of staff and a one month pilot test. The primary outcome was the decrease in VAP incidence rate and the process evaluation outcome were compliance and acceptability of the guideline, satisfaction and knowledge level of staff, and hospital cost reduction. The attainment of primary outcome and process evaluation outcome would be used to evaluate the effectiveness of the program. While some products (antimicrobial soaps, surface disinfectants, instrument disinfectants, wound antiseptics) may contain one or more biocidal agents with a comparable antimicrobial efficacy but large differences in their potential for microbial adaptation and tolerance. An increased bacterial resistance has been described for various antimicrobial agents, sometimes including a cross-resistance to antibiotics. Objective: Utilizing a multidisciplinary team approach to implement measures to improve the current care practices for nurses and respiratory therapist caring for mechanically ventilated patients in the Intensive Care Unit (ICU). Method: Improving the currently used ventilator care bundle to include; oral decontamination using Chlorhexidine Gluconate (CHG) every 12 hours, tooth brushing every 4 hours and the concurrent use of the accompanying oral care protocol which includes the use of an Oral Assessment Guide (OAG) and over the cuff/deep suctioning. Results: Initially the nurses and the respiratory therapist were not 100% compliant. However, through education using Evidence-Based Practice and incentives, compliance increased. The increase in compliance also decreased the incidence of VAP in the ICU. Conclusion: By utilizing a multidisciplinary team approach, education, documentation and EBP, the incidence of VAP was significantly reduced in a large trauma hospital in Arizona.

Developing a new oral care protocol is necessary to reduce or eliminate the development of this complication. Preventing or eliminating the development of ventilator-associated pneumonia will reduce hospital length of stay, decrease hospital cost, and decrease mortality rates which are caused by this illness. Research has been conducted on the use of chlorhexidine in routine oral care practice, which has shown to be effective in reducing or eliminating the development of ventilator-associated pneumonia. Chlorhexidine has been identified as an easy and inexpensive method in providing oral care with few adverse effects. Developing an appropriate implementation plan, while following the institutions policy and procedure, will provide and show the solution to addressing this complication. One barrier in implementing a change in oral care practice may be the nurses providing the care. Nurses view oral care on critically ill patients as a comfort measure and possibly compliancy can become an issue. Providing the education necessary to nursing staff is essential for the oral care practice to be successful. The aim of this proposal is to show key stakeholders the necessity for change and to implement a new oral care protocol. The proposal will have a written implementation plan explaining how this will be accomplished and an evaluation plan to obtain information in reference to the effectiveness of the proposal.

More than eighty percent of the worldwide deaths related to sepsis occur in resource-limited settings in low and middle-income countries. Current international sepsis guidelines cannot be implemented without adaptations towards these settings, mainly because of the difference in local resources and a different spectrum of infectious diseases causing sepsis. This prompted members of the Global Intensive Care working group of the European Society of Intensive Care Medicine (ESICM) and the Mahidol-Oxford Tropical Medicine Research Unit (MORU, Bangkok, Thailand) - among which the Editors – to develop with an international group of experts a comprehensive set of recommendations for the management of sepsis in resource-limited settings. Recommendations are based on both current scientific evidence and clinical experience of clinicians working in resource-limited settings. The book includes an overview chapter outlining the current challenges and future directions of sepsis management as well as general recommendations on the structure and organization of intensive care services in resource-limited settings. Specific recommendations on the recognition and management of patients with sepsis and septic shock in these settings are grouped into seven chapters. The book provides evidence-based practical guidance for doctors in low and middle income countries treating patients with sepsis, and highlights areas for further research and discussion.
