Currently being implemented throughout the CONUS, Tricare provides more equitable health care service to all military beneficiaries, improved access to care, a reduction in health care costs, and provides beneficiaries with an expanded choice of medical-care providers. This thesis examines the Tricare program and reviews relevant health care literature, both military and civilian. Using these inputs, the author presents a deterministic decision analysis model that allows a military beneficiary to select a health care option that minimizes his or her annual out-of-pocket costs while maintaining personal desires for choice among health care providers. Using several carefully selected examples that span the pool of military beneficiaries, the results of this study are presented. Every individual faced with the Tricare decision, approximately six million people, will gain insight from this thesis. While individual impact may only be on a scale of thousands of dollars, the impact for the entire pool of beneficiaries ranges well into the millions.

Chapter 55 of Title 10 U.S. Code, entitles certain health benefits to military personnel, retirees, and their families. These health benefits are administered by a Military Health System (MHS). The primary objectives of the MHS, which includes the Defense Department's hospitals, clinics, and medical personnel, are (1) to maintain the health of military personnel so they can carry out their military missions and (2) to be prepared to deliver health care during wartime. Health care services are delivered through either Department of Defense (DOD) medical facilities, known as military treatment facilities (MTFs) as space is available, or through civilian health care providers. As of 2017, the MHS operates 681 MTFs, employs nearly 63,000 civilians and 84,000 military personnel, and serves 9.4 million beneficiaries across the United States and in overseas locations. Since 1966, civilian care for millions of retirees, as well as dependents of active duty military personnel and retirees, has been provided through a program still known in law as the Civilian Health and Medical Program of the Uniformed Services (CHAMPUS), more commonly known as TRICARE. TRICARE has three main benefit plans: a health maintenance organization option (TRICARE Prime), a preferred provider option (TRICARE Select), and a Medicare supplement option (TRICARE for Life) for Medicare-eligible retirees. Other TRICARE plans include TRICARE Young Adult, TRICARE Reserve Select, and TRICARE Retired Reserve. TRICARE also includes a pharmacy program and optional dental and vision plans. Options available to beneficiaries vary by the sponsor's duty status and geographic location. Care Without Coverage examines the real consequences for adults who lack health insurance. The study presents findings in the areas of prevention and screening, cancer, chronic illness, hospital-based care, and general health status. The committee looked at the consequences of being uninsured for people suffering from cancer, diabetes, HIV infection and AIDS, heart and kidney disease, mental illness, traumatic injuries, and heart attacks. It focused on the roughly 30 million-one in seven-working-age Americans without health insurance. This group does not include the population over 65 that is covered by Medicare or the nearly 10 million children who are uninsured in this country. The main findings of the report are that working-age Americans without health insurance are more likely to receive too little medical care and receive it too late; be sicker and die sooner; and receive poorer care when they are in the hospital, even for acute situations like a motor vehicle crash.

Book excerpt: On April 1, 1997, a new Department of Defense plan for providing health care insurance for military personnel took effect at Ellsworth Air Force. This plan requires military retirees and their dependents to choose between two very different insurance options: Tricare Prime, which is a managed care Health Maintenance Organization, and Tricare Standard, which is a traditional Fee For Service plan. Both Prime and Standard have serious problems, and Tricare administrators did not provide eligible personnel with adequate information on which to base their choice of plan. This report examines background information on HMOs and military health care, the provisions of each Tricare option, and problems arising in their implementation in order to make an informed decision as to which plan provides better medical care. The two most important issues addressed are costs and medical care providers.Prime gives its enrollees very little control over management of their care and offers extremely limited choice of health care providers. It has not yet demonstrated its ability to provide quality care for serious illness, and it requires coordination of military and civilian providers in the same network. Tricare Standard is more expensive, but it offers beneficiaries control and extensive choices in management of their health care. It does not depend on civilian-military interaction, and does not subordinate retiree health care to military readiness issues. Therefore, for retirees living near Ellsworth, Standard would seem to offer higher quality care to those who can risk the potentially greater costs. It is, however, important to understand that changes in Tricare, pending federal legislation, military budgets, and response of local medical providers to problems with Tricare all require that any recommendation made now be frequently reevaluated.

Unfortunately, the experience of dying in the United States is often characterized by fragmented care, inadequate treatment of distressing symptoms, frequent transitions among care settings, and enormous care responsibilities for families. According to this report, the current health care system of rendering more intensive services than are necessary and desired by patients, and the lack of coordination among programs increases risks to patients and creates avoidable burdens on them and their families. Dying in America is a study of the current state of health care for persons of all ages who are nearing the end of life. Death is not a strictly medical event. Ideally, health care for those nearing the end of life harmonizes with social, psychological, and spiritual support. All people with advanced illnesses who may be approaching the end of life are entitled to access to high-quality, compassionate, evidence-based care, consistent with their wishes. Dying in America evaluates strategies to integrate care into a person- and family-centered, team-based framework, and makes recommendations to create a system that coordinates care and supports and respects the choices of patients and their families. The findings and recommendations of this report will address the needs of patients and their families and assist policy makers, clinicians and their educational and credentialing bodies, leaders of health care delivery and financing organizations, researchers, public and private funders, religious and community leaders, advocates of better care, journalists, and the public to provide the best care possible for people nearing the end of life.

Book excerpt: Social isolation and loneliness are serious yet underappreciated public health risks that affect a significant portion of the older adult population. Approximately one-quarter of community-dwelling Americans aged 65 and older are considered to be socially isolated, and a significant proportion of adults in the United States report feeling lonely. People who are 50 years of age or older are more likely to experience many of the risk factors that can cause or exacerbate social isolation or loneliness, such as living alone, the loss of family or friends, chronic illness, and sensory impairments. Over a life course, social isolation and loneliness may be episodic or chronic, depending upon an individual's circumstances and perceptions. A substantial body of evidence demonstrates that social isolation presents a major risk for premature mortality, comparable to other risk factors such as high blood pressure, smoking, or obesity. As older adults are particularly high-volume and high-frequency users of the health care system, there is an opportunity for health care professionals to identify, prevent, and mitigate the adverse health impacts of social isolation and loneliness in older adults. Social Isolation and Loneliness in Older Adults summarizes the evidence base and explores how social isolation and loneliness affect health and quality of life in adults aged 50 and older, particularly among low income, underserved, and vulnerable populations. This report makes recommendations specifically for clinical settings of health care to identify those who suffer the resultant negative health impacts of social isolation and loneliness and target interventions to improve their social conditions. Social Isolation and Loneliness in Older Adults considers clinical tools and methodologies, better education and training for the health care workforce, and dissemination and implementation that will be important for translating research into practice, especially as the evidence base for effective interventions continues to flourish.
