Book excerpt: January 1998 There is an apparent consensus that the correct health policy in developing countries is public provision of a mix of preventive and simple curative services through low level health workers and facilities. But the strength of this consensus on the primary health care paradigm is in sharp contrast to either the strength of its analytical foundations or its mixed record in practice. Filmer, Hammer, and Pritchett show how the recent empirical and theoretical literature on health policy sheds light on the disappointing experience with the implementation of primary health care. They emphasize the evidence on two weak links between government spending on health and improvements in health status. First, the capability of developing country governments to provide effective services varies widely-so health spending, even on the right services, may lead to little actual provision of services. Second, the net impact of government provision of health services depends on the severity of market failures. Evidence suggests these are the least severe for relatively inexpensive curative services, which often absorb the bulk of primary health care budgets. Government policy in health can more usefully focus directly on mitigating market failures in traditional public health activities and, in more developed settings, failures in the markets for risk mitigation. Addressing poverty requires consideration of a much broader set of policies which may-or may not-include provision of health services. This paper-a product of Poverty and Human Resources, Development Research Group-is part of a larger effort in the group to investigate efficacy in the social sectors. Book excerpt: When the United Nations announced the Millennium Development Goals (MDGs) in 2000, approximately half a million women worldwide died each year from complications associated with pregnancy and childbirth. The fifth MDG aimed to reduce the maternal mortality rate by 75 per cent between 1990 and 2015, but by the target date, the goal had not been reached. In The Limits of Trust Lisa Nicole Mills investigates the reasons why Mexico in particular did not meet its objective. Focusing on the states of Guerrero, Chiapas, and Oaxaca, where maternal mortality rates are the highest in the country, Mills looks into how MDG 5 has been implemented in Mexico, how it has been experienced by individuals and groups, what obstacles have been encountered, and what factors have facilitated improvements in maternal health. Using data gathered from interviews with NGOs, government officials, and health care workers, the book argues that government and feminist NGO efforts to build trust in the health care system have fallen short because of systemic failures to protect women’s rights and enhance the quality of health care. In Mexico a woman’s risk of dying from a pregnancy‐related complication is five times higher than in developed countries. The Limits of Trust explores the realities of implementing maternal health initiatives on the ground in rural, remote, and impoverished areas, and the steps that can be taken to successfully combat maternal mortality.

From the reforms that took place in Chile in 1981, most pension and health care systems in the region have seen reform, and been fully or partially privatized. Many other countries considering reform of their own systems have been influenced by the policies implemented in Latin America. Yet despite the importance and influence of these reforms, until now there has not been an integrated and comprehensive analysis of the changes and their effects. This book is the result of four years of painstaking work, data collection, field research and international collaboration, and so fills the vacuum in the literature with a systematic comparison of pension and healthcare reforms in the 20 Latin American countries. It identifies reform models, and elaborates taxonomies to facilitate their understanding and comparison. Some key features of the reforms to emerge are: labour force and population coverage, equity and solidarity, sufficiency and quality of benefits, state regulation, competition and degree of privatization, efficiency and administrative costs, social participation in management, financing sources and long-term sustainability. Effects of the reforms on social security principles are measured based on recent standardized statistics and other information. Goals or assumptions of the reforms are contrasted with actual outcomes, and the pros and cons of private versus private provision assessed. Detailed policy recommendations are offered to correct current problems and improve pension and healthcare systems.
