Book excerpt: The main objective of this book is to reduce endodontic failure, increase the success rate of endodontic treatment and decrease the percentage of tooth extraction so as to conserve a naturally compromised tooth structure as opposed to having a dental implant. The first study focuses on the possibility of having a filling material which could provide a tight seal with the desired properties. The in Vitro study targeted on elimination of microleakage by measuring the ability of the existing materials to obturate the root canal, 3 dimensionally using different obturation techniques. This was done by correlating the reading using the electrochemical machine and Micro CT Scan. This study adds value to the fact that the currently developed Nano HA based endodontic sealer is being produced locally in Malaysia thereby reducing the cost as compared to its imported counterpart. The third study consisted of testing the possibility of reinforcing over-flared root canal with Mineral trioxide aggregate (MTA) and also illustrated the pattern of root fracture. This development will definitely help economize the treatment thereby providing a good solution for young patients under 18 years of age.

Book excerpt: Shorttitle: The development and maintenance of apical seal is desirable and considered to be a major prerequisite to improve the outcome of root canal treatment. Basically, two materials are used in combination for the root canals filling; a semisolid material (gutta-percha) and a sealing cement. Many types and brands of endodontic sealers are commercially available, but none of them fully satisfy all requirements and able to provide a fluid- tight seal. To address this problem the School of Dental Sciences, USM, has prepared a new experimental nano HA-filled epoxy resin based endodontic sealer using a simple and inexpensive technique. Nano-HA is biocompatible, reactive and has capability to adhere to the dentinal tubules. Moreover, it promotes apical healing and is able to produce hermatic apical seal. Different filling techniques based on heated or preheated gutta-percha have been introduced to enhance three-dimensional filling of the root canal. Little is known regarding the apical sealing ability of the new experimental nano-HA sealer when used with different obturation techniques.

The major goals of root canal treatment are to clean and shape the root canal system and seal it in 3 dimensions to prevent reinfection of the tooth. Although initial root canal therapy has high degree of success, failures can still occur after treatment. Recent studies reported failure rates of 14%–16% for initial root canal treatment. Lack of healing is attributed to persistent intraradicular infection residing in previously uninstrumented canals, dentinal tubules, or in the complex irregularities of the root canal system. The extraradicular causes of endodontic failures include periapical actinomycosis, a foreign body reaction caused by extruded endodontic materials, an accumulation of endogenous cholesterol crystals in the apical tissues, and an unresolved cystic lesion. This book is composed of ten chapters and begins with the need of root-end resection. The succeeding chapters deal with the method and concept of root-end resection, root end preparation to receive the material, ideal requirements of materials to be placed in the resected and prepared root-end. The concluding chapters describe various root-end materials available in the market so far and their properties, focusing on each individual material one by one, their advantages and disadvantages. This book will prove useful to Endodontists and Researchers in the field of endodontic surgeries & root-end filling materials. Methacrylate resin-based sealers suffer from polymerization shrinkage stresses. This limitation may partly be overcome by a two-step cementation procedure. This alternative placement technique results in an increase and homogenization of the adhesion of the material to intraradicular dentin. Subsequent research should aim at developing sealers with shorter setting times for clinical implementation and confirm the present proof of concept. It remains to be proven whether an increase in adhesion could be considered as a surrogate for improved sealing ability. With the used methodology, we could not demonstrate any association between the sealing ability and the adhesiveness of an adhesive root canal filling material. Using the low surface tension fluid transport method, the sealing ability of the single-cone technique, after post space preparation, was found to be material dependent, with the epoxy and methacrylate resins providing a better seal than the calcium silicate-based sealer. Roots canals filled with a single-cone technique using gutta percha and a calcium silicate-based root canal sealer were less porous than root canals filled with lateral compaction of gutta percha and the same sealer. Delamination and tearing defects could be observed in root canals filled with hydrogel-coated hygro-expandable cones and a calcium silicate-based sealer. Hydrogels are smart materials that may currently be inadequate for endodontic use with calcium silicate sealers. The use of simulated body fluids for testing the bioactive potential of calcium silicate-based fillings leads to the deposition calcium phosphate. Book excerpt: O propósito deste estudo in vitro foi avaliar a qualidade de obturações endodônticas em relação ao selamento apical e marginal, comparando duas técnicas de obturação: Condensação Lateral (CL) e Condensação Vertical (CV) daguta-percha aquecida. Foram utilizadas 40 amostras que foram submetidas à penetração passiva de solução de nitrato de prata a 50% e divididas em dois grupos. Para cada grupo de amostras infiltradas foi utilizada uma metodologiadiferente. O grupo 1, com cada metade das amostras obturadas com um tipo de técnica de obturação, foi preparado para microscopia Óptica (MO), processamento digital de imagens (PDI) e espectrometria de absorção atômica (EAA). O grupo 2, com metade das amostras obturadas por cada uma das técnicas, foi preparado para microscopia eletrônica de varredura (MEV). Através da MO e PDI, as amostras foram avaliadas quanto à qualidade das obturações, quantificando o percentual de guta-percha e cimento presentes. Através da utilização da EAA, a massa de prata infiltrada na obturação foi quantificada e finalmente, com o uso da MEV, a adaptação marginal dos materiais obturadoresàs paredes do canal radicular foi avaliada. Nas condições experimentais em que este trabalho foi conduzido e através da análise estatística, concluiu-se que houve diferença estatisticamente significante em relação ao selamento apical produzido pelas duas técnicas de obturação avaliadas e quanto à qualidade de preenchimento das obturações. Nas observações ao MEV, a CV mostrou uma melhor adaptaçãodo material obturador às paredes dentinárias quando comparada com a CL em algumas regiões do canal radicular.
