Book excerpt: Introduction : En France, lors d'une indication de déclenchement artificiel, les prostaglandines E2 sont recommandées en première intention, pourtant elles semblent influencer négativement le vécu de la naissance. Objectifs : Evaluer la satisfaction des femmes concernant la prise en charge de leur déclenchement par prostaglandines E2 et décrire les paramètres influençant positivement leur satisfaction afin de l'optimiser. Population et méthode : Etude épidémiologique descriptive transversale auprès de 105 femmes déclenchées par prostaglandines E2 dans une maternité de type III, à l'aide de questionnaires. Résultats : Le score de satisfaction général était de 4,0 [4,0 - 5,0]. L'information et l'accompagnement avaient un impact significativement modéré sur la satisfaction générale, alors que celui de la prise en charge de la douleur était significativement faible. L'adaptation de l'information à la réalité et l'implication des femmes, ainsi que la disponibilité des sages-femmes semblaient être les points clefs de cette satisfaction. Discussion : Pour optimiser cette satisfaction, il parait nécessaire d'évaluer l'information donnée aux femmes selon le personnel médical et de l'adapter si besoin aux données scientifiques mais aussi d'étudier l'efficacité d'alternatives afin d'accroitre le sentiment d'implication des femmes. Connaitre la vision des sages-femmes sur cette prise en charge pourrait aussi permettre de dégager des axes d'amélioration. Conclusion : Les femmes étaient satisfaites de la prise en charge de leur déclenchement, mais des études supplémentaires pourraient permettre d'améliorer le vécu de la naissance de leur enfant dans le contexte d'un déclenchement, où la prise en charge psychologique par la sage-femme semble primer sur la prise en charge physique.

Aujourd'hui, cette technique protocolisée fait l'objet de recommandations nationales émises par l'HAS. Les méthodes pharmacologiques (ocytociques, prostaglandines) sont de loin les plus utilisées devant les méthodes mécaniques (décollement du pôle inférieur de l'oeuf, double-ballonnet, dilapans). Ces dernières présentent a priori moins d'effets secondaires et un coût réduit pour une efficacité comparable (sources Cochrane et HAS). Leur réalisation en pratique courante est cependant entravée par le manque d'études à leur sujet. Objectif : Evaluer et comparer l'efficacité et les répercussions de deux méthodes de déclenchement du travail à terme sur col perméable : prostaglandines E2 versus double-ballonnet. Méthode : Etude comparative rétrospective effectuée à l'hôpital Croix-Rousse et l'hôpital Femme Mère Enfant des Hospices Civils de Lyon, de Janvier à Septembre 2012 et incluant au total 151 parturientes (80 femmes déclenchées par PGE2 et 71 femmes déclenchées par double-ballonnet).Principaux résultats : Notre étude ne montre pas de différence significative entre la méthode déclenchement par prostaglandines E2 et celle par double-ballonnet concernant l'échec de maturation (p=0,14), le gain sur le score de Bishop (p=0,58), le mode d'accouchement (p=0,95), le risque de décollement placentaire (p=1), l'hémorragie du post-partum (p=1), l'adaptation à la vie extra-utérine et l'état hypoxique foetal. La méthode par double-ballonnet est plus à risque de remontée de la présentation foetale (p=0,008) et de recours aux ocytociques (p

Along with these changes has come a growing concern to adopt means and methods which could make termination easier, safer and more effective. In this evolution, termination in the second trimester in par~icular is notable as being responsible for a dis proportionate share ofthe complications and adverse experiences associated with pregnancy termination. Although the almost universal shift towards earlier abortion has reduced the number of second trimester procedures as a percentage of the whole, the problems of interrupting pregnancy in the second trimester remain conspicuous. Delay in either seeking or obtaining abortion is still, in many parts of the world, all too frequent. AdditiDnally, recent developments in the prenatal diagnosis of fetal malformations, alphafeto protein screening programmes and changes in the pattern of and approaches to intrauterine fetal death now also place greater emphasis on the need for adequate methods of interrupting pregnancy in the second trimester. Unlike the first trimester in which vacuum aspiration is universally con sidered to be the method of choice, in the second trimester of pregnancy the clinician is faced with alternatives; one method may be more appropriate than another in a particular circumstance and no single method is unequivocall~ accepted as best. Neither do second trimester terminations form a neatly defined single category.
