Book excerpt: The gases discussed in the Conseil's study are those most often used in hospitals and other workplaces such as dental and veterinarian offices, namely halothane, nitrous oxide, a combination of halothane and nitrous oxide, enflurane and isoflurane. In order to evaluate the effects of exposure on reproduction, animal and epidemiological studies were examined. Nineteen epidemiological studies, from which we retained 24 comparisons between occupationally exposed and non-exposed women, were included in a meta-analysis. Overall, it was calculated that the risk of miscarriage is 51% higher in exposed women. Restrictingg the analysis to the most valid studies increased the risk even further (up to 86%). The epidemiological data on birth defects were also examined, but they did not show any consistent pattern of increased risk. Although the majority of available data concerned women working in environments without gas scavenging systems, our assessment prompts caution. In brief, occupational exposure of pregnant women to anesthetic gases to levels higher than established norms should be avoided. Appropriate scavenging systems should have been installed and their good functioning regularly monotored.

Fetal well-being is an additional risk-benefit factor to be considered when weighing options before conducting non-obstetric surgeries on pregnant patients.
