Despite these challenges, self-regulated learning allows trainees to learn continuously, and motor skills development can be augmented through mental practice and motor imagery. The aim of this chapter is to introduce the theoretical concepts in skill acquisition and the role of mental and deliberate practice as an alternative for skill training. A case study is presented using a design and development framework for producing an online basic micro suturing training resource based on self-regulated learning. This case study demonstrates the use of the ADDIE instructional design model and Mayer,Äôs multimedia theory guidelines, for creating online instructional resources. The methodological approach of a design and developmental framework to create an educationally sound online training module for micro suturing which has significant utility in hand surgery is discussed in this chapter. The tools described in this chapter are translatable to any psychomotor skills development in medical education.

Book excerpt: "A physical surgical simulator, the McGill Inanimate System for Training and Evaluation of Laparoscopic Skills (MISTELS) is used to examine the acquisition of laparoscopic skills, as well as to determine a summative evaluation of these skills. The cumulative summation method (CUSUM) is used to characterize individual and group learning curves for a MISTELS task. To examine transfer of learning in laparoscopy, novice laparoscopists were randomized to either practice a basic psychomotor task (peg board transfer) or to a no-practice control group. After forty iterations, the novices performed a more complex task (suturing); those who practiced the basic task significantly improved their complex task performance. Finally, using the receiver operator curve (ROC), the summative evaluation score for MISTELS was determined by maximizing the sensitivity and specificity of this training instrument. Using these findings, individualized training programs can be developed to teach essential laparoscopic skills prior to entering the operating room." --

The frequency of individual accidents - usually lost-time injuries - does not predict the likelihood of an organizational accident. The book also elaborated upon the widely-cited Swiss Cheese Model. Organizational Accidents Revisited extends and develops these ideas using a standardized causal analysis of some 10 organizational accidents that have occurred in a variety of domains in the nearly 20 years that have passed since the original was published. These analyses provide the ’raw data’ for the process of drilling down into the underlying causal pathways. Many contributing latent conditions recur in a variety of domains. A number of these - organizational issues, design, procedures and so on - are examined in close detail in order to identify likely problems before they combine to penetrate the defences-in-depth.
