Book excerpt: This investigation report examines an explosion and fire that occurred on October 13, 2002, at the First Chemical Corporation facility in Pascagoula, Mississippi. The decomposition of mononitrotoluene inside a distillation column resulted in a runaway reaction and explosion, with potentially catastrophic offsite conse- quences. This report identifies the root and contributing causes of the incident and makes recommendations on evaluating reactive hazards, applying lessons learned, layers of protection, work prac- tices, facility siting, and community notification. The U.S. Chemical Safety and Hazard Investigation Board (CSB) is an independent Federal agency whose mission is to ensure the safety of workers, the public, and the environment by investigating and preventing chemical incidents. CSB is a scientific investigative organi- zation; it is not an enforcement or regulatory body. Established by the Clean Air Act Amendments of 1990, CSB is responsible for determining the root and contributing causes of accidents, issuing safety recommendations, studying chemical safety issues, and evaluating the effectiveness of other government agencies involved in chemical safety. No part of the conclusions, findings, or recommendations of CSB relating to any chemical incident may be admitted as evidence or used in any action or suit for damages arising out of any matter men- tioned in an investigation report (see 42 U.S.C. § 7412 [r][6][G]). CSB makes public its actions and decisions through investigation reports, summary reports, safety bulletins, case studies, incident digests, special technical publications, and statistical reviews. Book excerpt: On March 23, 2005, at 1:20 p.m., the BP Texas City Refinery suffered one of the worst industrial disasters in recent U.S. history. Explosions and fires killed 15 people and injured another 180, alarmed the community, and resulted in financial losses exceeding $1.5 billion. The incident occurred during the startup of an isomerization (ISOM) unit when a raffinate splitter tower was overfilled; pressure relief devices opened, resulting in a flammable liquid geyser from a blowdown stack that was not equipped with a flare. The release of flammables led to an explosion and fire. All of the fatalities occurred in or near office trailers located close to the blowdown drum. A shelter-in-place order was issued that required 43,000 people to remain indoors. Houses were damaged as far away as three-quarters of a mile from the refinery.

(CTA) facility in Corbin, Kentucky, killed seven and injured 37 workers. This incident caused extensive damage to the production area of the 302,000-square-foot plant. Nearby homes and an elementary school were evacuated, and a 12-mile section of Interstate 75 was closed. The largest CTA customer, Ford Motor Company, temporarily suspended operations at four automobile assembly plants because CTA had produced acoustic insulation products for those plants, as well as for other industrial and automotive clients. In investigating this incident, the U.S. Chemical Safety and Hazard Investigation Board (CSB) determined that combustible phenolic resin dust that had accumulated throughout the facility fueled the explosion.

Book excerpt: On February 7, 2008, at about 7:15 p.m., a series of sugar dust explosions at the Imperial Sugar manufacturing facility in Port Wentworth, Georgia, resulted in 14 worker fatalities. Eight workers died at the scene and six others eventually succumbed to their injuries at the Joseph M. Still Burn Center in Augusta, Georgia. Thirty six workers were treated for serious burns and injuries—some caused permanent, life altering conditions. The explosions and subsequent fires destroyed the sugar packing buildings, palletizer room, and silos, and severely damaged the bulk train car loading area and parts of the sugar refining process areas. The Imperial Sugar manufacturing facility housed a refinery that converts raw cane sugar into granulated sugar. A system of screw and belt conveyors, and bucket elevators transported granulated sugar from the refinery to three 105-foot tall sugar storage silos. It was then transported through conveyors and bucket elevators to specialty sugar processing areas and granulated sugar packaging machines. Sugar products were packaged in four-story packing buildings that surrounded the silos, or loaded into railcars and tanker trucks in the bulk sugar loading area. The U.S. Chemical Safety and Hazard Investigation Board (CSB) determined that the first dust explosion initiated in the enclosed steel belt conveyor located below the sugar silos. The recently installed steel cover panels on the belt conveyor allowed explosive concentrations of sugar dust to accumulate inside the enclosure. An unknown source ignited the sugar dust, causing a violent explosion. The explosion lofted sugar dust that had accumulated on the floors and elevated horizontal surfaces, propagating more dust explosions through the buildings. Secondary dust explosions occurred throughout the packing buildings, parts of the refinery, and the bulk sugar loading buildings. The pressure waves from the explosions heaved thick concrete floors and collapsed brick walls, blocking stairwell and other exit routes. The resulting fires destroyed the packing buildings, silos, palletizer building and heavily damaged parts of the refinery and bulk sugar loading area.
