Book excerpt: La réussite d’un traitement orthodontique dépend de plusieurs facteurs, la force que l’on applique aux organes dentaires est un des éléments mais ce n’est pas tout; il ne faut pas omettre l’importance des fonctions. Parmi les fonctions oro-faciales, la déglutition, de par sa fréquence : 2400 par 24 h tient une place primordiale dans l’étiopathogénie des malocclusions. De plus, la durée d’application des forces exercées par la langue estimée à 22h sur 24, conforte l’idée du rôle de la fonction linguale dans le développement des malocclusions. La déglutition atypique entraîne des répercussions au niveau de la croissance du massif facial, de la phonation, de la respiration, de l’occlusion et peut entraîner des malpositions dentaires. Ainsi, en présence d’une déglutition dite dysfonctionnelle, la réussite ainsi que la stabilité du traitement orthodontique sont menacées ; une rééducation préalable de cette déglutition est nécessaire pour prévenir les récidives. Après avoir défini la déglutition fonctionnelle, nous présenterons les caractéristiques de la déglutition dysfonctionnelle et nous évoquerons son étiologie et ses conséquences chez l’adolescent ainsi que chez l’adulte. Enfin, nous finirons par les différents traitements que l’on peut proposer à ces patients dont la fonction de déglutition est perturbée..

En effet, la langue est un muscle puissant qui a une place capitale entre les arcades dentaires. Sa dysfonction peut entrainer des béances sous différentes formes. Ces béances posent un problème occlusal, c'est-à-dire au niveau de l'engrènement des arcades dentaires. Le concept centrage-calage-guidage est altéré. De plus, en présence d'une béance au niveau du secteur incisif, le guidage antérieur est absent. Nous pouvons alors nous demander quels sont les grands principes occlusaux à respecter lors d'une réhabilitation fonctionnelle et esthétique. Aussi, s'il est nécessaire d'avoir recours à d'autres spécialistes tels que les orthodontistes ou les orthophonistes . Afin de répondre à ces questions, nous allons tout d'abord faire quelques rappels d'anatomie, de physiologie et d'occlusion. Puis, nous verrons les étapes indispensables dans le plan de traitement de ces cas complexes. Enfin, nous illustrerons nos propos par des cas cliniques.

Book excerpt: Although speech-language pathologists are expected to be able to administer and interpret oral examinations, there are currently no screening tests available that provide careful administration instructions and data for intra-examiner and inter-examiner reliability. The Oral Speech Mechanism Screening Examination (OSMSE) is designed primarily for use by clinical speech-language pathologists. The examination could also serve, however, as a useful tool for oral myofunctional therapists, as well as physical therapists, dentists, and orthodontists interested specifically in speech. It is reliable, relatively easy and quick to administer, and appropriate for children and adults in either diagnostic or therapy settings. The OSMSE is intended to assess those anatomical structures and physiological functions that are most often considered to be potentially related to speech or language disorders. Structural and/or functional judgments included are organized on the OSMSE test form under categories of: lips, tongue, jaw, teeth, hard palate, soft palate, pharynx, breathing, and diadochokinesis. Results of a preliminary study with normal speaking subjects indicated that the OSMSE had satisfactory intra- and inter-examiner reliability after minimal training. The time required to administer the examination typically ranged from 5 to 10 minutes. (Author/GK)
