Book excerpt: Understanding the complexities of kidney illness is essential to encouraging a healthy lifestyle, since kidney health is a crucial aspect of overall well being. This chapter provides readers with an intelligent introduction to the complex world of the kidneys and acts as the starting point for a thorough investigation of renal health. Fundamentally, kidney disease is a deterioration of renal function that, if untreated, may result in major health problems. This introductory chapter provides readers with a foundation of information that helps them understand why kidney health is so important. The chapter opens with a brief synopsis of kidney illness, giving readers a clear idea of what it involves. It highlights the vital function of the kidneys in the body, which include controlling blood pressure, electrolyte levels, and fluid balance in addition to acting as waste filters. This introduction highlights the kidneys' hidden heroes in preserving internal balance and highlights their importance in the complex dance of body processes. Expanding upon this basis, the chapter delves into the significance of renal health. It clarifies how having healthy kidneys helps avoid a number of health problems and emphasizes the importance of healthy kidneys for maintaining a balanced and healthy body. Book Effects of Renal Denervation Vs Sham in Resistant Hypertension After Medication Escalation

Download or read book Effects of Renal Denervation Vs Sham in Resistant Hypertension After Medication Escalation written by Michel Azizi and published by . This book was released on 2022 with total page 0 pages. Available in PDF, EPUB and Kindle. Book excerpt: Abstract: Importance Although early trials of endovascular renal denervation (RDN) for patients with resistant hypertension (RHTN) reported inconsistent results, ultrasound RDN (uRDN) was found to decrease blood pressure (BP) vs sham at 2 months in patients with RHTN taking stable background medications in the Study of the ReCor Medical Paradise System in Clinical Hypertension (RADIANCE-HTN TRIO) trial. Objectives To report the prespecified analysis of the persistence of the BP effects and safety of uRDN vs sham at 6 months in conjunction with escalating antihypertensive medications. Design, Setting, and Participants This randomized, sham-controlled, clinical trial with outcome assessors and patients blinded to treatment assignment, enrolled patients from March 11, 2016, to March 13, 2020. This was an international, multicenter study conducted in the US and Europe. Participants with daytime ambulatory BP of 135/85 mm Hg or higher after 4 weeks of single-pill triple-combination treatment (angiotensin-receptor blocker, calcium channel blocker, and thiazide diuretic) with estimated glomerular filtration rate (eGFR) of 40 mL/min/1.73 m2 or greater were randomly assigned to uRDN or sham with medications unchanged through 2 months. From 2 to 5 months, if monthly home BP was 135/85 mm Hg or higher, standardized stepped-care antihypertensive treatment starting with aldosterone antagonists was initiated under blinding to treatment assignment. Interventions uRDN vs sham procedure in conjunction with added medications to target BP control. Main Outcomes and Measures Six-month change in medications, change in daytime ambulatory systolic BP, change in home systolic BP adjusted for baseline BP and medications, and safety. Results A total of 65 of 69 participants in the uRDN group and 64 of 67 participants in the sham group (mean [SD] age, 52.4 [8.3] years; 104 male [80.6%]) with a mean (SD) eGFR of 81.5 (22.8) mL/min/1.73 m2 had 6-month daytime ambulatory BP measurements. Fewer medications were added in the uRDN group (mean [SD], 0.7 [1.0] medications) vs sham (mean [SD], 1.1 [1.1] medications; P = .045) and fewer patients in the uRDN group received aldosterone antagonists at 6 months (26 of 65 [40.0%] vs 39 of 64 [60.9%]; P = .02). Despite less intensive standardized stepped-care antihypertensive treatment, mean (SD) daytime ambulatory BP at 6 months was 138.3 (15.1) mm Hg with uRDN vs 139.0 (14.3) mm Hg with sham (additional decreases of −2.4 [16.6] vs −7.0 [16.7] mm Hg from month 2, respectively), whereas home SBP was lowered to a greater extent with uRDN by 4.3 mm Hg (95% CI, 0.5-8.1 mm Hg; P = .03) in a mixed model adjusting for baseline and number of medications. Adverse events were infrequent and similar between groups. Conclusions and Relevance In this study, in patients with RHTN initially randomly assigned to uRDN or a sham procedure and who had persistent elevation of BP at 2 months after the procedure, standardized stepped-care antihypertensive treatment escalation resulted in similar BP reduction in both groups at 6 months, with fewer additional medications required in the uRDN group. Trial Registration ClinicalTrials.gov Identifier: NCT02649426

Book Essential Manual of 24 Hour Blood Pressure Management

Download or read book Essential Manual of 24 Hour Blood Pressure Management written by Kazuomi Kario and published by John Wiley & Sons. This book was released on 2022-02-01 with total page 400 pages. Available in PDF, EPUB and Kindle. Book excerpt: ESSENTIAL MANUAL OF 24-HOUR BLOOD PRESSURE MANAGEMENT Hypertension is one of the greatest threats to human health. The World Health Organization (WHO) estimates that 1.13 billion people worldwide have hypertension. In 2017, new guidelines for managing hypertension were published by the American Hypertension Association (AHA), guidelines which lowered the diagnosis thresholds of hypertension, and thereby increased the prevalence of hypertension. As such, hypertension is now recognized as a more serious and widespread a condition than ever before. In this new edition of the Essential Manual of 24-Hour Blood Pressure Management, the author emphasizes that lowering the blood pressure (BP) and restoring the BP profile with adequate circadian rhythm is essential for a long life without cardiovascular events. The author also introduces updated evidence for managing hypertension throughout 24-hour periods, from morning to nocturnal hypertension. The Essential Manual of 24-Hour Blood Pressure Management, Second Edition, will be an essential companion for doctors who wish to provide evidence-based medicine and be familiar with the most cutting edge technology on monitoring BP. Medical researchers and students will also value the author’s many insights, drawn from his distinguished career.

Book Renal Denervation

  • Author : Richard R. Heuser
  • Publisher : Springer
  • Release : 2014-11-11
  • ISBN : 1447152239
  • Pages : 203 pages

Download or read book Renal Denervation written by Richard R. Heuser and published by Springer. This book was released on 2014-11-11 with total page 203 pages. Available in PDF, EPUB and Kindle. Book excerpt: Hypertension remains the leading cause of cardiovascular morbidity and mortality in spite of current medical therapies. It has been estimated that 50% of Western civilization has hypertension and approximately 20% of patients have resistant hypertension. Renal denervation (RDN) is a minimally invasive, endovascular catheter based procedure using radiofrequency ablation aimed at treating resistant hypertension. Early studies show a high degree of effectiveness in renal denervation to treat hypertension. This book examines renal pathophysiology and the rationale for renal denervation, as well as possible long term benefits and risks of this new therapy. The myriad of devices involved in the evolution of this therapy are discussed and the book concludes with analyses of the cost effectiveness and future applications.

Book Function of Renal Sympathetic Nerves

Download or read book Function of Renal Sympathetic Nerves written by Yutang Wang and published by Frontiers Media SA. This book was released on 2017-10-18 with total page 98 pages. Available in PDF, EPUB and Kindle. Book excerpt: Sympathetic overactivity is associated with the development of hypertension. Renal denervation (RDN) prevents or delays hypertension in a variety of animal models, which laid the groundwork for the introduction of RDN as a clinical therapy in humans. In 2007, a novel, minimally invasive RDN ablation catheter was first trialled in hypertensive patients, with a 93% success rate of lowering blood pressure for at least three years post-RDN. However, a large scale, sham-controlled clinical trial (Symplicity HTN -3) failed to show reductions in BP greater than sham. The aim of this research topic was to evaluate the efficacy and safety of RDN, to explore the contribution of both afferent and efferent renal nerve activity to hypertension and non-hypertension disorders, and to stimulate future research to better understand the function of the renal nerves and the effects of RDN by highlighting gaps in knowledge.

Book Kidney Inflammation Injury and Regeneration

Download or read book Kidney Inflammation Injury and Regeneration written by Patrick C. Baer and published by MDPI. This book was released on 2020-04-03 with total page 496 pages. Available in PDF, EPUB and Kindle. Book excerpt: Acute kidney injury (AKI) is still associated with high morbidity and mortality incidence rates, and also bears an elevated risk of subsequent chronic kidney disease. Although the kidney has a remarkable capacity for regeneration after injury and may recover completely depending on the type of renal lesions, the options for clinical intervention are restricted to fluid management and extracorporeal kidney support. The development of novel therapies to prevent AKI, to improve renal regeneration capacity after AKI, and to preserve renal function is urgently needed. The Special Issue covers research articles that investigated the molecular mechanisms of inflammation and injury during different renal pathologies, renal regeneration, diagnostics using new biomarkers, and the effects of different stimuli like medication or bacterial components on isolated renal cells or in vivo models. The Special Issue contains important reviews that consider the current knowledge of cell death and regeneration, inflammation, and the molecular mechanisms of kidney diseases. In addition, the potential of cell-based therapy approaches that use mesenchymal stromal/stem cells or their derivates is summarized. This edition is complemented by reviews that deal with the current data situation on other specific topics like diabetes and diabetic nephropathy or new therapeutic targets.
