Book excerpt: Contexte : Des travaux récents soulignent l’importance de l’évaluation angiographique des lésions coronaires complexes multiples, qui sont associées à une déstabilisation multifocale des plaques d’athérome et à une augmentation des évènements cardiovasculaires à court et long terme. Par ailleurs, d’autres études suggèrent que les parodontopathies pourraient être un facteur important dans le développement des maladies cardiovasculaires, et également dans la déstabilisation des plaques d’athérome par l’inflammation. But : Le but de notre étude est de déterminer s’il existe, chez un patient ayant présenté un infarctus du myocarde, une relation entre la sévérité de la parodontopathie et la sévérité de la coronaropathie. Matériels et méthodes : Etude prospective sur 150 patients admis en réadaptation cardiaque ambulatoire pour un infarctus du myocarde datant de moins d’un mois et ayant bénéficié d’une coronarographie. L’évaluation de l’atteinte parodontale est réalisée grâce à un panoramique dentaire qui permet l’analyse de 5 paramètres : l’inflammation, la mastication, l’édentation, les caries, et la perte osseuse. La coronarographie permet l’identification des lésions complexes définies par une sténose supérieure à 50%, et au moins 2 critères parmi l’occlusion chronique, l’altération de flux, la présence d’un thrombus, l’irrégularité, ou l’ulcération. Les patients sans lésions complexe ou avec une lésion complexe (groupe lésion complexe simple (LCS)) ont été comparés aux patients avec plus d’une lésion complexe (groupe lésions complexes multiples (LCM)). Résultats : Après infarctus, 21% des patients (32/150) appartiennent au groupe LCM. Les deux groupes ne présentent pas de différence significative au niveau de leurs facteurs de risque cardiovasculaires. Les patients du groupe LCM sont plus souvent multitronculaires que les patients du groupe LCS (88% versus 46%, p 0.001). Les patients du groupe LCM ont une CRP supérieure aux patients du groupe LCS (CRP 10 mg/l : 42% versus 15% p = 0.001). Les patients du groupe LCM ont plus fréquemment une perte osseuse parodontale supérieure à 50% que les patients du groupe LCS mais les résultats sont non significatifs (50% versus 32%, p =0.063). Les autres paramètres d’évaluation bucco dentaires sont comparables. En analyse multivariée une perte osseuse parodontale supérieure à 50% augmente significativement la relation avec les LCM (OR (95%CI) 2.63 (1.03-6.71) p = 0.043)) même après ajustement sur les autres facteurs connus prédictifs de LCM (CRP et atteinte multitronculaire). Conclusion : La perte osseuse est un paramètre simple et objectif d’évaluation de l’atteinte parondontale, prédictif du caractère complexe et multiple des lésions coronaires, de façon indépendante des autres facteurs connus, et permet ainsi d’identifier les patients les plus à risque d’évènements cardio vasculaires.
