Book excerpt: D'après le postulat sur la « théorie de la guidance », la racine de l'incisive latérale maxillaire guide l'éruption de la canine maxillaire qui lui est associée. L'objectif de cette étude rétrospective est donc de rechercher l'existence d'une relation de cause à effet entre l'inclusion canine et l'agénésie de l'ILM ou une modification de son volume. Nous avons analysé 53 radiographies panoramiques, selon des méthodes précises tirées de la littérature, de patients recrutés au sein de l'unité fonctionnelle d'orthodontie et de pédodontie du CHU de Nantes, de Paris et de cabinets orthodontiques privés. Ces patients sont répartis en trois groupes : 31 patients dans le groupe agénésie, 17 dans le groupe microdontie et 5 dans le groupe macrodontie. Nous avons pu identifier 32% de risque d'inclusion canine dans le groupe agénésie, et 12,5% dans le groupe microdontie. Comparé à la fréquence d'inclusion de la canine maxillaire dans la population générale (1 à 3%) nous pouvons conclure à un lien significatif entre l'agénésie ou la microdontie de l'ILM et l ‘inclusion de la canine maxillaire associée. La faible fréquence d'intervention chirurgicale chez les patients à risque, par la mise en place d'une interception précoce et efficace, nous pousse à une démarche simple de dépistage que pourrait constituer les analyses radiographiques utilisées dans cette étude.
