Book excerpt: Introduction : L'activité physique rythme notre quotidien et influe sur notre qualité de vie. Objectif : L'objectif de cette enquête épidémiologique est de rechercher les facteurs associés à la dimension physique du score de qualité de vie chez le chirurgien-dentiste et notamment si la pratique d'une activité physique améliore cette qualité de vie. Méthode : Au cours de l'année 2015 (avril à juillet), un questionnaire anonyme recueillant des informations concernant la localisation, la pratique, le mode de vie, l'activité physique, sportive et la qualité de vie liée à la santé (SF-12) a été administré à certains chirurgiens-dentistes français. Résultats : 923 sujets ont été inclus. Les réponses obtenues ont révélé que l'activité sportive influait positivement sur le score de qualité de vie liée à la santé avec un facteur proportionnel à sa fréquence. Elle a aussi montré que le jogging, l'absence d'antécédents médicaux et les dispositifs de correction visuelle augmentaient significativement le score de qualité de vie liée à la santé. Au contraire le stress et les douleurs musculaires avaient un impact négatif proportionnel à leur fréquence. L'activité d'omnipratique, la consultation d'un kinésithérapeute, d'un psychiatre/psychologue et d'un ORL diminuaient le score de qualité de vie liée à la santé de l'individu. Discussion : Suite aux résultats de l'étude et conformément à notre hypothèse, chez le chirurgien-dentiste, il est apparu que la pratique d'une activité physique et en particulier le jogging, était liée au score physique de la qualité de vie liée à la santé, après ajustement avec les autres facteurs généraux connus pour l'influencer.

De nombreuses études épidémiologiques se sont intéressées à l'étude de la relation activité physique et qualité de vie, que se soit en population générale ou chez des sujets atteints de maladie chronique. Cependant, ces études ont principalement été menées chez les adultes et très peu d'entre elles ont étudié cette relation lorsque l'activité physique était pratiquée au cours d'une hospitalisation. Objectif. L'objectif général était d'approfondir les connaissances sur la relation activité physique et qualité de vie liée à la santé chez les enfants et adolescents en population générale et en milieu hospitalier.Méthode. Nous avons répondu à cet objectif général au travers de trois programmes de recherche. Tout d'abord, grâce aux données d'inclusion de l'essai REGUL'APS mené en septembre 2005, essai randomisé en grappe chez des enfants scolarisés du CE1 au CM2 en région Lorraine. Ensuite, en utilisant les données de l?étude APOP (Activités Physiques en Population Pédiatrique), essai randomisé en cross-over répliqué, chez des enfants âgés de 5 à 18 ans, atteints d'un cancer et hospitalisés dans le cadre de leur traitement dans le service de Médecine Infantile II (hémato-oncologie/ immunologie) de l'Hôpital d'Enfants du CHU de Nancy. Enfin, nous avons mis en place l'essai Activ'Hop, dans le but d'étudier cette relation chez les enfants hospitalisés dans d'autres services hospitaliers de l'hôpital d'Enfant de Nancy, tels que les services de Médecine Infantile I et III (pédiatrie générale, génétique) et de Chirurgie Orthopédique/Brûlés. Résultats. Nos résultats reflètent qu'une pratique d'activité physique élevée est associée à une meilleure qualité de vie liée à la santé chez les enfants scolarisés en école primaire. En particulier, les garçons présentent une meilleure qualité de vie lorsqu'ils pratiquent une plus grande variété d'activités physiques. Les résultats d'APOP montrent qu'une pratique d'activité physique adaptée durant l'hospitalisation améliore la qualité de vie liée à la santé des enfants atteints d'un cancer dans la plupart des dimensions physique et psychologique. En ce qui concerne l'essai Activ'hop, dont sa mise en place est présentée et discutée dans le manuscrit, les enfants sont actuellement en cours d'inclusion.Conclusion. Au cours de ce travail de thèse, nous avons confirmé l'impact positif à court terme qu'une pratique d'activité physique sur la qualité de vie des enfants et adolescents, qu'elle se fasse dans la vie de tous les jours ou au cours d'une hospitalisation. Aussi, il est nécessaire de maintenir ou de développer les moyens mis en place pour que les enfants et adolescents puissent faire de l'activité physique et ce quelque soit leur état de santé et/ou leur condition de vie.

La mise au point des instruments de mesure d'activité physique (QAPE-semaine) et de qualité de vie (PedsQLTM 4.0) était un préalable indispensable. Chez les adultes, la pratique d'une activité physique de loisirs est associée à une meilleure qualité de vie, tant dans une approche transversale (dimensions physiques et mentales) que longitudinale (dimension mentale). Un essai randomisé en grappe chez les enfants d'âge scolaire a permis de montrer qu'une même quantité d'éducation physique et sportive pratiquée à l'école ne produisait pas de différence de qualité de vie ou de vitesse d'augmentation de l'indice de masse corporelle, quel que soit le mode de fractionnement et de répartition dans la semaine. La réalisation d'un essai randomisé permet d'obtenir des résultats convaincants, malgré les difficultés organisationnelles de ces essais en milieu scolaire qui sont discutés. Ils confirment la discordance avec les études observationnelles dans lesquelles un lien entre corpulence et fractionnement de l'activité physique peut être mis en évidence, sans qu'il soit possible de faire la part entre la quantité et la répartition dans la semaine. L'ensemble des résultats, leur implication pour la recherche et la santé publique sont discutés.

Those familiar with Henri Savall’s and his colleague Véronique Zardet’s earlier work on the socio-economic theory of organizations will recognize their assessments of organizational dysfunctions and hidden costs – but in a different context. In their current work, the emphasis is on the tensions created by the wider environment – the idea of tetranormalization – and how those tensions shape and influence organizational life. Drawing on a wide range of examples from the news media and popular press, Savall and Zardet paint a disturbing picture of the underlying dynamics and challenges posed by a literal avalanche of standards and norms – which are often ambiguous and conflicting – that literally encompasses all that we do. Approaches as different as S-O-R behaviorism,social learning theory, consistency theories,and expectancy value theories of motivation and decision making have one thing in common: they all assume that "thought (or any other type of cognition) can move any thing," that there is a direct path from cognition to behavior. In recent years, we have become more and more aware of the complexities in volved in the relationship between cognition and behavior. People do not always do what they intend to do. Aside from several nonpsychological factors capable of reducing cognition-behavior consistency, there seems to be a set of complex psychological mechanisms which intervene between action-related cognitions, such as beliefs, expectancies, values, and intentions,and the enactment of the be havior suggested by those cognitions. In our recent research we have focused on volitional mechanismus which presumably enhance cognition-behavior consistency by supporting the main tenance of activated intentions and prevent them from being pushed aside by competing action tendencies.
