L'âge moyen de diagnostic est de 61 ans. Trente pourcents des patientes doivent bénéficier d'une mastectomie totale. Le taux de reconstruction mammaire (RM) tout âge confondu est de 22,9%. Objectif : l'objectif principal de notre étude est d'évaluer la population opérable et la population opérée en termes de RM chez les femmes de 70 ans et plus ayant bénéficié d'une mastectomie. Matériels et méthodes : il s'agit d'une étude rétrospective multivariée unicentrique incluant les patientes de 70 ans et plus ayant bénéficié d'une mastectomie entre 2005 et 2018 au sein de l'Institut Bergonié. La population a été répartie en 2 groupes : patientes avec ou sans RM. Pour chaque groupe, nous avons décrit les données démographiques et médicales, le taux de propositions de reconstructions (chirurgien ou patiente), le taux de refus émanant du chirurgien ou de la patiente, le degré de satisfaction concernant la réalisation ou non d'une RM. Résultats : 578 patientes ont été traitées par mastectomie. 59 patientes ont bénéficié d'une RM (10,21%). 262 patientes ont été incluses, 59 avec reconstruction et 201 sans reconstruction. Les facteurs influençant positivement la RM étaient l'âge inférieur à 75 ans, le statut OMS2, le score G814, l'absence de traitement anticoagulant, la mastectomie pour carcinome canalaire in situ, la proposition faite par le chirurgien. Les patientes avec RM exprimaient une satisfaction à hauteur de 87,5%. Les causes de regret étaient la lourdeur de la chirurgie, la présence de complications, le résultat esthétique. Chez les patientes sans RM, 43,8% n'avaient pas reçu de proposition de leur chirurgien, 38,8% ont émis un refus, avec pour principales causes le sentiment d'âge dépassé, la lassitude médicale, la peur de l'intervention et l'absence de besoin. 25,87% des patientes non reconstruites exprimaient un regret. Conclusion : malgré l'âge avancé, de nombreuses patientes expriment une volonté de RM. Il est important de les informer des différentes techniques chirurgicales ainsi que de leurs possibles complications afin d'établir une population cible, éligible à la reconstruction et de limiter le nombre de regrets.
