L'augmentation de la RM coïncide avec l'élargissement de l'indication de la radiothérapie post-mastectomie (PMRT). Cependant, la PMRT augmente les complications liées à la RM par prothèse. L'Institut national du cancer (INCA) suggère qu'une bonne sélection des patientes sur la base de leurs comorbidités et de leurs caractéristiques oncologiques permet la réalisation d'une RM immédiate (RMI) sans prendre de risque carcinologique. Nous avons essayé de comparer la satisfaction à long terme de la RMI prothétique par rapport à la RM différée (RMD). Matériel et méthode : cette étude observationnelle rétrospective a été menée de janvier 2010 à décembre 2020 au Centre Antoine Lacassagne à Nice. Les questionnaires BREAST-Q ont été envoyés pour évaluer la satisfaction. Résultats : nous avons inclus 108 patients ayant bénéficié d'une RM prothétique et d'une PMRT. 33 patients ont répondu au questionnaire. Les questions concernant la reconstruction d'un schéma corporel correspondant au standard féminin, l'esthétique et la satisfaction globale n'étaient pas significatives mais en faveur de la RMI. A la question « au cours des 14 derniers jours, avez-vous été satisfaite de pouvoir porter des vêtements plus serrés ? » 72,8% des patientes du groupe RMI ont répondu " tout à fait satisfait " contre 31,8% dans le groupe RMD (p : 0.086). 63,7% des patientes en RMI étaient "assez satisfaites” du volume du sein reconstruit contre 31,8 % des patientes en RMD (p : 0,087). Les questions sur l'inconfort ont révélé plus de douleurs chroniques chez les patientes avec RMI que chez les patientes avec RMD (54,5% vs 36,4% (p : 0,62) pour les douleurs musculaires thoraciques). Les complications immédiates et tardives étaient similaires avec 20,9% de coque de grade 2 ou plus pour les patientes en RMI et 20,1% pour les patientes en RMD. Conclusion : notre étude semble confirmer l'intérêt de la RMI même en cas de PMRT au vu des taux de complications qui n'entraînent pas de retard à la prise en charge adjuvante, à condition que les patients soient bien sélectionnés. Ces résultats, ainsi qu'une meilleure satisfaction des patientes en terme d'esthétique de la reconstruction, de gain de qualité de vie et d'estime de soi en RMI, incitent à développer cette pratique.
