Book excerpt: INTRODUCTION : Les carcinomes épidermoïdes (CE) de la cavité buccale sont fréquents, de mauvais pronostic, souvent localement avancés au diagnostic. Le traitement repose sur une chirurgie première souvent invasive. La voie d'abord chirurgicale doit permettre une exposition optimale et une résection en bloc. L'objectif était de comparer les résultats carcinologiques et à long terme selon la voie d'abord, dans les T3-T4a de la langue mobile et du plancher buccal.METHODE : Cette étude rétrospective incluait les patients opérés de ces tumeurs de 1998 à 2016 à l'Institut Gustave Roussy. Les données épidémiologiques, la voie d'abord, le curage, la reconstruction, le traitement adjuvant éventuel, étaient recueillis. Les résultats anatomopathologiques, la survie globale (SG) et sans récidive (SR), le contrôle local (CL) étaient analysés. Les complications post-opératoires, à long terme, le résultat fonctionnel et la qualité de vie (QdV) étaient évalués.RESULTATS : Parmi les 239 patients inclus, 125 ont eu une voie de pull-through (PT), 67 une voie transmandibulaire (TM) et 47 une voie buccale (EB). Les patients opérés par voie EB étaient plus âgés (p=0,002) et avaient des tumeurs localement moins avancées (p=0,007). Book excerpt: La prise en charge des tumeurs du plancher buccal et de la base de langue dépend en grande partie de la classification TNM. Cependant celle-ci ne tient pas compte de l'infiltration tumorale dans les tumeurs classées T1-3. Alors que toute tumeur solide a une évolution tridimensionnelle. Le but de notre étude rétrospective était d'étudier l'influence de l'épaisseur ou l'infiltration tumorale sur le pronostic dans le carcinome épidermoide de la cavité buccale. PATIENTS ET METHODE : L'étude a inclus 124 patients T1-3 N0M0. L'âge moyen était de 59,3 ans avec un ratio hommes/femmes de 3,4. Ont été analysés les différentes caractéristiques épidémiologiques, puis les différents facteurs pronostiques incluant l'infiltration tumorale. RESULTATS : Hormis le sex-ratio, les données épidémiologiques correspondaient aux données de la littérature. Les probabilités de survie globale à 2 et 5 ans étaient de 85,6 % et 77,3 %, respectivement et inférieures aux données de la littérature. Les facteurs pronostiques étaient l'envahissement ganglionnaire, la taille tumorale et l'infiltration tumorale. CONCLUSION : Notre étude a montré, avec une taille suffisante, une homogénéité certaine et avec un recul moyen supérieur à la majorité des études, que la probabilité de survie globale à 5 ans diminue sensiblement lorsque l'infiltration tumorale est supérieure à 6,5 mm. En attendant une évaluation pré-thérapeutique, en routine de l'infiltration tumorale des cancers du plancher buccal et de la langue mobile par échographie ou par IRM, nous recommandons l'intégration de l'infiltration tumorale dans la stratégie thérapeutique, notamment pour les tumeurs classées T1, la nécessité d'effectuer un évidement cervical lorsque l'infiltration tumorale maximale atteint ou dépasse 6,5 mm et de s'en abstenir en dessous de 3 mm. La question reste posée concernant l'attitude à avoir vis-à-vis des tumeurs ayant une infiltration maximale entre 3 et 6,5 mm.
