Le choix de ce sujet est une volonté personnelle de faire la lunière sur ce fléau et sur sa prise en charge, après nous être questionnés durant nos études face à des cas équivoques. Cette thèse a plusieurs objectifs. Tout d'abord, nous ferons le point sur l'épidémiologie de la rnaltraitance afin de mieux repérer les enfants en danger. Dans une deuxième partie, nous nous pencherons sur les diffièrentes fomes cliniques que peuvent prendre les sévices afin de faciliter leur identification par le chirurgien-dentiste. Ensuite, nous préciserons le cadre légal pour ma prévention et le signalement .Enfin, dans une dernière partie, nous présenterons notre étude qui a pour but de cemer les problérnatiques rencontrées pat· les chirurgiens dentistes face à la maltraitance sur enfants, et de donner des pistes de réflexions.

Quatre types de maltraitance à enfant sont généralement décrits : la négligence, les abus physiques, la violence psychologique et les abus sexuels. Le chirurgien-dentiste est obligé par la loi mais également par le code de déontologie de signaler toute suspicion de maltraitance. Celui-ci est idéalement placé pour la détecter. En effet, il est en relation avec une proportion non négligeable d'enfants dans sa patientèle. De plus, de nombreux signes cliniques de la maltraitance se situent dans la région oro-faciale et peuvent être dépistés en premier lieu par le chirurgien-dentiste. La conduite à tenir au cabinet dentaire doit être clairement définie pour être efficace. Elle dépendra de la situation de l'enfant en danger : l'enfant en risque de maltraitance, l'enfant maltraité en dehors d'une situation d'extrême gravité et l'enfant maltraité en situation d'extrême gravité. Ce travail vise à récapituler les démarches à suivre dans le cadre d'une procédure de signalement, ainsi que les différentes instances à contacter. Malheureusement, peu de chirurgiens-dentistes font les démarches nécessaires pour de multiples raisons. L'effort demandé est minime mais peut sauver une vie.
