Book excerpt: L'accroissement de la fréquentation des services d'urgence impose aux soignants d'imaginer de nouveaux modes de fonctionnement. Nous avons évalué au sein du service d'accueil des urgences du centre hospitalier de Libourne une organisation basée sur un médecin coordinateur des flux, dont le rôle est de garder une vision d'ensemble du service, de centraliser les informations, de coordonner les actions. Il est sollicité par l'Infirmière Organisatrice de l'Accueil pour une aide à l'orientation et à l'évaluation, pour la prescription rapide d'antalgiques et pour la prise en charge précoce de patients nécessitant un engagement limité du plateau technique. L'objectif est la réduction des temps de passages aux urgences et l'amélioration de la qualité des soins. 400 dossiers de patients ont été étudiés sur deux périodes (avec et sans médecin coordinateur). Ont été comparés : les délais de prise en charge et les délais de prescriptions d'antalgiques rapportés aux catégories défibies par les classifications CCMU et GEMSA ainsi que les niveaux de satisfaction des patients et des soignants. Cette analyse confirme que la présence du médecin coordinateur diminue le temps de passage global aux urgences de 12 % (surtout lié à la précocité du contact médical) principalement pour les patients classés G2N2 (sortant après une consultation) et sans augmenter les délais pour les patients G4N3 (hospitalisés, nécessitant un traitement en urgence, mais stables). La prescription d'antalgiques est plus importante et plus rapide avec un médecin coordinateur et l'activité de soins de ce praticien représente 18,9 % des passages. Ce dispositif, qui devra être réévalué,, permet donc : - une diminution des temps de passage aux urgences - une amélioration de la qualité de prise en charge des patients - une optimisation du traitement de la douleur.
