Sleep deprivation is a major cause for concern among health care professionals, with ICU patients being at highest risk. Nurses play a key role in developing, implementing, and disseminating changes to improve patients' sleep. A quiet environment offers a healing environment. "A review of literature providing an hour of uninterrupted rest during the day, noise levels decreased from 55% to 39%" (Boehm and Morast, 2009, pages 3). Significant research supports quiet time protocols in the Intensive Care Unit improve patient outcomes, decrease ventilator days and length of stay, and prevent the incidence of ICU delirium. However, culture change is needed to endorse such protocols. The intent of this capstone project was to provide critical care nurses with evidence-based practice and education about quiet time in the ICU environment. Invoking interest and motivation for change will assist in forming a culture that can implement and sustain a nurse driven protocol in the ICU that will overall benefit patient outcomes.

Book excerpt: Critical care environments are not conducive to providing ample opportunities for patients to rest and sleep; lack of sleep or sleep deprivation, are often linked to noisy, chaotic, bright environments. Sleep deprivation is noted to be a cause of diagnosis with ICU delirium that is linked to increased morbidity and mortality rates and increased length of hospital stay (Devlin, Brummel and Al-qadheeb, 2012). Research supports simple solutions such as dimming lights and decreasing noise levels to improve patients' sleep cycles. In addition to positive patient outcomes, instituting a quiet time protocol has also been shown through research to positively affect nursing staff. A study by Riemer, Mates, Ryan and Schleder (2015), found that decreasing lighting and sound throughout a critical care unit, was shown to decrease nurses' stress levels in a statistically significant way. Implementation of a quiet time program will begin with education of staff and project stakeholders to create buy-in for the necessity to change current practice. Use of a champion team will be implemented to educate, intervene, collect data and disseminate findings throughout the critical care and to key stakeholders. Evaluations of a control (pre-intervention) group and intervention group, nursing surveys and chart reviews will all be implemented to collect data and determine effectiveness of instituting a quiet time program in critical care. With positive findings, dissemination of this program can begin and be adapted outside of critical care for added benefit to the hospitalized patients and nursing staff alike.
