Each policy brief is adapted from the executive summary of its corresponding report. Details of each study are available in the full report cited at the end of each policy brief. The case studies were carried out to inform the summary report, which was developed jointly with the OECD under WHO Kobe Centre’s leadership as one of the flagship technical products of WHO designated in its Thirteenth General Programme of Work, 2019–2023.

Book excerpt: This scoping review aimed to identify alternative purchasing arrangements (apart from fee for service [FFS] and pay for performance [P4P]) and their associated service delivery models that countries have used to improve the quality of chronic care, as well as to assess how these purchasing arrangements have impacted the quality of care. A scoping review of academic and grey literature published between 2013 and 2022 was carried out on alternative purchasing arrangements and their impact on the quality of chronic care using causal inference methods. The analytical approach consisted of clustering articles around similar purchasing arrangements or programmes, summarizing the evidence across the clusters, and highlighting the service delivery aspects and institutional design features that potentially impacted the quality of care. This research provided information to the WHO and OECD joint publication Purchasing for quality chronic care: summary report.

Book excerpt: There has been significant interest globally in understanding how purchasing arrangements can be designed strategically to optimize the delivery of health services. Given the growing burden of chronic diseases worldwide and the costs they are likely to impose on health systems, it is imperative that health services for chronic diseases are purchased in a way that contributes to the objectives of universal health coverage, including improved quality, and ultimately achieves better health outcomes. This paper explores the role of purchasing arrangements in improving the quality of chronic disease care. Specifically, the paper provides an overview and summary of the types of payment methods that have been used, summarizes evidence of their effectiveness in improving the quality of chronic care and assesses the enablers and barriers to the implementation of purchasing reforms. For that matter, a scoping review of the peer-reviewed literature was conducted, focusing on payment methods that seek to improve the quality of chronic disease care published. The lessons from these payment initiatives implemented to improve the quality of chronic disease care, mainly from HICs, suggest that they are a promising tool but, nonetheless, the quality and equity aspects need more attention in terms of the design of such initiatives as well as in terms of evidence generation. The significant information technology infrastructure and resources required to implement such arrangements potentially limit their applicability in low-resource settings.

This study aims to explore the impact of purchasing arrangements for the National Basic Public Health Services Programme on the quality of care for chronic illnesses and analyse the factors that facilitate or inhibit its impact.

Book excerpt: This report was developed to inform the WHO Technical Product, purchasing instruments to strengthen quality health services for chronic illnesses (WHO-OECD joint publication). The Australian primary care system largely consists of independent private medical practices funded by fees for services. A Health Care Homes model that included voluntary patient enrolment and voluntary provider participation, with bundled payment for chronic disease management was implemented as a trial from October 2017 to June 2021. The model aimed to provide more flexible services delivered by multidisciplinary teams. Previous attempts to introduce patient enrolment supported by capitation had mixed results. The trial design of this involved a variety of stakeholders. Practices did not become purchasers of other services, and careful attention was paid to identifying and risk-rating suitable patients. The evaluation of the trial demonstrated some improved disease management and more use of a variety of practitioners without any significant change in hospital use. The decrease in fee-for-service revenue was more than compensated for by the capitated payments.

Care Without Coverage examines the real consequences for adults who lack health insurance. The study presents findings in the areas of prevention and screening, cancer, chronic illness, hospital-based care, and general health status. The committee looked at the consequences of being uninsured for people suffering from cancer, diabetes, HIV infection and AIDS, heart and kidney disease, mental illness, traumatic injuries, and heart attacks. It focused on the roughly 30 million-one in seven-working-age Americans without health insurance. This group does not include the population over 65 that is covered by Medicare or the nearly 10 million children who are uninsured in this country. The main findings of the report are that working-age Americans without health insurance are more likely to receive too little medical care and receive it too late; be sicker and die sooner; and receive poorer care when they are in the hospital, even for acute situations like a motor vehicle crash.
