The anomaly is clinically significant because the tubercle that fractures easily exposes the pulp which in turn leads to pulpal necrosis and periapical pathology. It is a condition that merits early recognition and treatment in order to prevent complication. A case of successful management of dens evaginatus with fractured tubercle and pulpal necrosis is reported.
