Book excerpt: Percutaneous Transluminal Coronary Angioplasty is presently the widest used non-surgi cal method for the treatment of stenotic lesions in coronary artery disease. Continuous development of the procedure and equipment has made complex and multi-vessel-inter ventions possible and has led to other techniques of intravascular angioplasty. In reviewing current results, this volume pays particular attention to causes, incidence, circumstances, recognition, treatment and outcome of complications of PTCA; it also considers restenosis, which still occurs at an unsatisfactory high rate after primarily successful procedures. To deal with these topics, experts on PTCA met to discuss their experiences and insights into peculiar aspects of complications in PTCA. The discussion was subdivided into: Recognition of complications; Complications in new interventional techniques; Management of complications; and Restenosis after angioplasty. The question of whether angiographic and pathological characteristics of sclerotic lesions allow the identification of complication-prone lesions in advance is discussed in the first chapter. One paper deals with the state of the art of digital cardiac imaging, which enables the operator to have an early knowledge of and adequate reaction to possible complications occurring during angioplasty. Special attention is directed to the so-called "high-risk"-PTCA, detailing the clinical variables, in order to have better knowledge about patients with a higher probability of unsuccessful or complicated procedures.

Book excerpt: Obstruction of coronary blood flow and the resultant consequences are the center stage pathophysiologic events in cardiology today. The speculations of Jenner, Burns, Heberdin, McKenzie, Prinzmetal and many others had until now been left to observations of isolated tissue and intact animal experimentation. Only with the advent of Gruentzig's technique, which allowed us to 'work safely inside the coronary arteries' are we able to observe the effects of coronary occlusion in living conscious man. PTCA provides not only a therapeutic modality for non-operatively opening coronary obstructions, but has also provided the best model for studying the effects of acute ischemia on the heart. The procedure also lead the way to all other interventional cardiology developments, including modern thrombolysis in the setting of acute myocardial infarction. In his previous works, Serruys has examined how PTCA can serve as a model for studying acute ischemia. In this book, he and his co-authors discuss the effects of balloon-induced ischemia on the electrocardiographic changes, coronary blood flow dynamics, cardiac muscle metabolism and left ventricular function, as well as measures to counter these effects and provide for reperfusion in unstable angina and acute myocardial infarction. Technology has expanded the 'eyes' of the observer of these events. The authors use many techniques including ECG recording from surface, endocardium, and intracoronary electrodes; angiographic assessment of coronary flow pattern using digital techniques, as well as doppler flow measurements; biochemical assessment of metabolic products stimulated by ischemia; and digital angiographic and echo doppler assessment of left ventricular function.

The meeting described in this book was held under the auspices of the working group of our national society, chaired by Prof. Kaltenbach, Frankfurt, and by Prof. Meyer, Mainz, in cooperation with Stanford University. It is an attempt to present various cardiologists' appraisals of the current position ofPTCA in clinical medicine. As a physiologist: "What is the effect of this stenosis on coronary blood flow and myocardial function?"; as a clinician: " Is this lesion responsible for the patient's symptoms?"; and finally as an interventionalist: "Will revascularization of this artery improve the patient?" Fundamentally, the answer to these questions can be given to a large extent by measuring coronary pressure. That is the rationale of writing this book. 1. 1 Historical overview. Andreas Gruentzig and most interventional cardiologists in the early days of PTCA, had the intuitive feeling that pressure measurements could help to establish the severity of a coronary stenosis and to monitor the progress and result of a coronary intervention. At that time, measuring coronary pressure by the balloon catheter was part of a standard procedure. A residual transstenotic gradient of less than 15 mmHg was generally considered as a good result. Later, however, it turned out that measuring these (resting) gradients with balloon catheters was inaccurate an only had a limited prognostic value. Moreover, because there was no consistent theory to correlate pressure measurements to blood flow, the interest in measuring coronary pressures faded and disappeared almost completely with the introduction of new balloon catheters not intended for pressure measurement.
