Emerging evidence suggests that neurobiological factors are not only critical in providing potential risk factors but also provide a promising approach to develop more effective treatment and prevention strategies. The Neurobiological Basis of Suicide discusses the most recent findings in suicide neurobiology. Psychological, psychosocial, and cultural factors are important in determining the risk factors for suicide; however, they offer weak prediction and can be of little clinical use. Interestingly, cognitive characteristics are different among depressed suicidal and depressed nonsuicidal subjects, and could be involved in the development of suicidal behavior. The characterization of the neurobiological basis of suicide is in delineating the risk factors associated with suicide. The Neurobiological Basis of Suicide focuses on how and why these neurobiological factors are crucial in the pathogenic mechanisms of suicidal behavior and how these findings can be transformed into potential therapeutic applications.

Book excerpt: An examination of the efficacy and safety of psychiatric medications in light of how little is understood about how they work It is estimated that forty-five to fifty percent of all Americans will suffer a mental disorder at some time during their lives. Increasingly, the treatment for these disorders is management with one or more psychiatric drugs, often prescribed by general practitioners. In Pillaged Ronald William Maris evaluates the psychiatric medications commonly used to treat several major types of psychiatric disorders—-including depression and mood disorders, bipolar disorders, anxiety disorders, and psychotic disorders—asking "do they work as advertised?" and, more importantly, "are they safe?" Answers to these questions are more ambiguous than we might think, Maris explains, because drug manufacturers tend to minimize the adverse effects of their products. Furthermore, the underlying neurobiological theories of how psychiatric drugs work are complex, poorly understood, and often conflicting. Still Americans spend tens of billions of dollars a year on antidepressants and antipsychotics alone. While Maris questions the rampant prescribing of psychiatric medications especially in young people, Pillaged does not suggest that anyone cavalierly discontinue potentially beneficial psychiatric medications without the advice of a qualified mental health professional. The book acknowledges that psychiatric medications are often necessary in treating some psychiatric conditions, but it reminds readers of medication's potential for degrading one's quality of life, contributing to self-destructive behaviors, and even leading to death in a vulnerable minority of patients. Maris advocates an open and honest discussion of data on psychiatric drugs, their effects, and their dangers, and he reminds readers of available alternative, nondrug treatments for psychiatric disorders. By reviewing the history and effects of medications for mental disorders, Maris hopes to educate health care consumers and prescribers to make careful, informed decisions about the treatment of psychiatric disorders.

Book excerpt: The primary aim of this study was to examine the relationship between components of suicidal desire and psychotropic medication. Specifically, the usage of psychotropic medication, the usage of specific classes of psychotropic medications and the amount of psychotropic medication utilized and differences in feelings of perceived burdensomeness, thwarted belongingness and overall suicide risk were examined. The present study utilized pre-collected data consisting of 225 patients with substance use disorder undergoing residential treatment for substance dependence. It was posited that individuals utilizing psychotropic medications would exhibit higher mean levels of thwarted belongingness, perceived burdensomeness, and suicide risk relative to individuals not utilizing psychotropics and that individuals utilizing multiple psychotropic medications would exhibit higher suicidal desire and risk than individuals utilizing zero or one psychotropic medication. Additionally, it was posited that individuals utilizing antipsychotics would exhibit higher suicidal desire and risk than individuals utilizing any other type of psychotropic medication. For all hypotheses it was posited that such effects would occur above and beyond severity of psychopathology and substance use. Results indicated a significant difference in suicide risk between individuals utilizing a psychotropic and individuals not utilizing a psychotropic. Additional exploratory analyses indicated a significant difference in levels of perceived burdensomeness between individuals utilizing an antidepressant and individuals utilizing any other type of psychotropic besides an antidepressant. Overall, results suggest that the use of psychotropic medication may increase the risk for suicide. These findings highlight the need for routine suicide risk assessments for patients utilizing psychotropic medications.

These disorders are among the most highly stigmatized health conditions in the United States, and they remain barriers to full participation in society in areas as basic as education, housing, and employment. Improving the lives of people with mental health and substance abuse disorders has been a priority in the United States for more than 50 years. The Community Mental Health Act of 1963 is considered a major turning point in America's efforts to improve behavioral healthcare. It ushered in an era of optimism and hope and laid the groundwork for the consumer movement and new models of recovery. The consumer movement gave voice to people with mental and substance use disorders and brought their perspectives and experience into national discussions about mental health. However over the same 50-year period, positive change in American public attitudes and beliefs about mental and substance use disorders has lagged behind these advances. Stigma is a complex social phenomenon based on a relationship between an attribute and a stereotype that assigns undesirable labels, qualities, and behaviors to a person with that attribute. Labeled individuals are then socially devalued, which leads to inequality and discrimination. This report contributes to national efforts to understand and change attitudes, beliefs and behaviors that can lead to stigma and discrimination. Changing stigma in a lasting way will require coordinated efforts, which are based on the best possible evidence, supported at the national level with multiyear funding, and planned and implemented by an effective coalition of representative stakeholders. Ending Discrimination Against People with Mental and Substance Use Disorders: The Evidence for Stigma Change explores stigma and discrimination faced by individuals with mental or substance use disorders and recommends effective strategies for reducing stigma and encouraging people to seek treatment and other supportive services. It offers a set of conclusions and recommendations about successful stigma change strategies and the research needed to inform and evaluate these efforts in the United States.

Also, the daily growing number of papers on suicide among schizophrenic are certainly a more updated source of information may contribute to the reduction of deaths by suicide among these patients. Yet, as in the case of suicide as a whole, this progress of knowledge does not match with reduction of suicide rates, let alone reduction of suicide rates among people with schizophrenia. Maybe a summary, an overview that cannot be achieved with a simple Medline search may help those who are involved and those who should be involved in the prevention of self-killing of schizophrenic patients. This book, therefore, reports essays of some of the opinion leaders in the field with the aim to shed light to such overwhelming phenomenon.

During the past 20 years, 25 states and the District of Columbia have legalized cannabis and/or cannabidiol (a component of cannabis) for medical conditions or retail sales at the state level and 4 states have legalized both the medical and recreational use of cannabis. These landmark changes in policy have impacted cannabis use patterns and perceived levels of risk. However, despite this changing landscape, evidence regarding the short- and long-term health effects of cannabis use remains elusive. While a myriad of studies have examined cannabis use in all its various forms, often these research conclusions are not appropriately synthesized, translated for, or communicated to policy makers, health care providers, state health officials, or other stakeholders who have been charged with influencing and enacting policies, procedures, and laws related to cannabis use. Unlike other controlled substances such as alcohol or tobacco, no accepted standards for safe use or appropriate dose are available to help guide individuals as they make choices regarding the issues of if, when, where, and how to use cannabis safely and, in regard to therapeutic uses, effectively. Shifting public sentiment, conflicting and impeded scientific research, and legislative battles have fueled the debate about what, if any, harms or benefits can be attributed to the use of cannabis or its derivatives, and this lack of aggregated knowledge has broad public health implications. The Health Effects of Cannabis and Cannabinoids provides a comprehensive review of scientific evidence related to the health effects and potential therapeutic benefits of cannabis. This report provides a research agendaâ€"outlining gaps in current knowledge and opportunities for providing additional insight into these issuesâ€"that summarizes and prioritizes pressing research needs.
