Book excerpt: As a researcher whose work focuses largely on the causes and conse quences of unwanted pregnancy, I may appear to be an unlikely candidate to write a foreword to a book on infertility. Yet, many of the themes that emerge in the study of unwanted pregnancy are also apparent in the study of infertility. Moreover, this volume is an important contribution to the literature on fertility, women's health issues, and health psychology in general, all topics with which I have been closely involved over the past two decades. Neither pregnancy nor its absence is inherently desirable: The occurrence of a pregnancy can be met with joy or despair, and its absence can be a cause of relief or anguish. Whether or not these states are wanted, the conscious and unconscious meanings attached to pregnancy and in fertility, the responses of others, the perceived implications of these states, and one's expectations for the future all are critical factors in determining an individual's response. In addition, both unwanted pregnancy and failure to conceive can be socially stigmatized, evoking both overt and subtle social disapproval. Fur ther, they involve not only the woman, but her partner, and potentially the extended family. Finally, both of these reproductive issues have been poorly researched. Because both are emotionally charged and socially stigmatized events, they are difficult to study. Much of the early literature relied on anecdotal or case reports.

Book excerpt: Infertility is a health problem that affects approximately 48.6 million couples worldwide. The affective response towards infertility may have consequences on women’s overall mental and physical health. The surrounding/contextual cultural factors (e.g. social pressure, religion, gender roles) can affect the meaning that people give to infertility, which can result in psychological distress in a woman who is infertile due to not achieving a desired social role and social expectations. Objectives: Examine whether infertility-related stress domains from the Fertility Problem Inventory, anxiety, and depression differ between infertile women from collectivistic and individualistic cultures; and assess whether infertility-related stigma and social pressure to have children are endorsed more from infertile women in the collectivistic group than from those in the individualistic group. Method: A cross-sectional study with a total of 59 women who completed an online survey that was posted in several infertility forums and that was sent out to email list serves from June to November 2017. Results: Analyses of covariance showed that infertility-related stress domains of social concern, rejection of a childfree lifestyle, and need for parenthood were significantly higher in the individualistic than the collectivistic group. Anxiety and depression levels did not differ between groups. Most women in this study endorsed feeling stigma and/or social pressure, both of which may be influenced by social expectations, older age, and being around others who have children, but neither stigma nor social pressure differed between groups. Conclusions: Based on results from this study, it appears that women from individualistic cultures might experience more infertility-related stress than women from collectivistic cultures. These results should be interpreted with caution, however, as the sample size was small and not representative of the general population with regard to education level and age. Nonetheless, these results can be applied to medical settings to help clinicians better understand the psychosociocultural factors that the infertility experience can render.
