Stress may hasten HIV disease progression by compromising immune response and increasing viral replication. Conversely, efforts to reduce stress may buffer the physiological effects of stress on HIV disease progression. Purpose. This study determined the effects of a structured, eight-week, mindfulness-based stress reduction (MBSR) program on perceived stress, mood, stress hormone secretion, immune function, and functional health outcomes in HIV-infected subjects. Conceptual framework. A synthesized framework of psychoneuroimmunology and a transactional model of stress guided the study. Design. A quasi-experimental, repeated measures, control group design was utilized. Subjects were nonrandomly assigned to intervention (N = 24) or control group (N = 10) and data was collected at baseline, four weeks, eight weeks, and three months postintervention (only baseline and eight weeks for control). Data Analysis. Paired t-tests between baseline scores and scores from subsequent time points and repeated measures ANOVAs were used to determine within group changes. Independent t-tests (calculated on change scores from baseline to eight weeks) were used to determine between group differences. Pearson correlation coefficients were calculated between change scores for psychological, endocrine, and immune measures to determine the associations among these variables. Results. At eight-weeks, the MBSR group showed significant increases in total functional health score, natural killer (NK) cell activity, NK cell number, and a significant decrease in fatigue. In addition, RANTES levels significantly increased while levels of SDF-1 remained stable. At three months postintervention, significant increases in NK cell number and function were maintained, and significant decreases in tension, depression, anger, confusion, and total mood disturbance emerged. Control subjects showed no changes in any dependent variables. Serum cortisol and DHEAS did not significantly change in either group. Within MBSR group changes over time (ANOVA) were found for perceived stress, tension, depression, anger, confusion, fatigue, and NK cell number. Between group differences in change scores were found for NK cell activity and number. No significant associations were found among change scores for dependent variables. Conclusion. These results provide empirical evidence that MBSR may assist in slowing HIV disease progression and may be considered an effective adjunctive therapy in the comprehensive management of HIV disease.

Book excerpt: Background: The immunological as well as the psycho-social impact, of living with HIV/AIDS transform HIV/AIDS into a multidimensional process. Stigma and discrimination against people living with HIV/AIDS (PLWHA) are proposed as hostile scenarios increasing hopelessness and reducing perceived and real social support affecting people's health status. Peer support strategies are proposed as key factors for dealing with this scenario; additionally, socio-cultural variables may determine the provision and perception of social support. Objectives: To enhance the understanding of the process of living with HIV/AIDS and the role played by social support and to suggest cooperative strategies for dealing with stigma and discrimination against PLWHA to improve people's health. Sample and method: Five studies were conducted studying 37 HIV positive members and non-members of peer support organisations (PSOs) in Chile and England; nine healthcare professionals working with PLWHA; and three spokes persons from PSOs of PLWHA from Romania, England and Chile. Results: PSOs of PLWHA, which reflect a cooperative strategy used by PLWHA to deal with stigma and self-provide social support, appear to play an important and underexplored role in PLWHAs' health status; this relationship is also affected by socio-cultural characteristics. A measure of PSS was developed and theoretical analysis lead to a linkage with Maslow's hierarchy of needs. Personality characteristics were found critical for the success of PNI based interventions. Conclusions: Living with HIV/AIDS involves psychological and social complications. PSOs are a powerful cooperative strategy improving quality of life and general health; however, further research is needed to establish the real impact of PSOs over HIV+ people. Implications: The peer-support strategy of PSOs is a powerful but underused clinical strategy. Healthcare teams and PLWHA may benefit from including this strategy if cooperative work is carried out with PSOs.

Book excerpt: The National Institute of Mental Health (NIMH) AIDS Program is the fourth largest acquired immune deficiency syndrome (AIDS) program within the National Institutes of Health (NIH). Since 1983, our program's contributions have concentrated on two major areas. The first has been to develop effective strategies to prevent or reduce behaviors that place individuals at risk for human immunodeficiency virus type 1 (HIV-1) infection. The second has been to support and foster research to enhance our understanding of the profound impact of H IV-1 infection on the central nervous system (eNS). The brain appears to be a prime target of the virus and may serve as a reservoir for the virus. Post mortem examination of brain tissue has provided evidence of eNS cell damage in 80%-90% of people who die with AIDS. For about 10%-20% of people with AI DS, mild neuropsychological symptoms are the first signs of the disease. Approximately 20% of individuals infected with HIV-1 develop AIDS dementia complex. The symptoms include apathy, difficulty concentrat ing, irritability, depression, and personality changes. In the later stages of the disease, people may experience psychiatric disor ders. Death usually occurs within 6 months of the appearance of those more severe symptoms. Up to 90% of children infected with H IV-1 experience attention and concentration difficulties and often experience neurodevelopmental delay or regression over time.
