KIMBALL BACKGROUND It was observed in 1930 that seminal plasma of a number of species contained very large amounts of substance(s) capable of altering uterine motility. These substances were subsequently demonstrated to be prostaglandins (PGs); this began the early and continued association of this group of compounds with reproduction. The activity was found to be associated with a fraction containing lipid-soluble acids, derived from prostanoic acid. Restrictions imposed by obscure sources and small amounts of material available for testing were initially impediments to their study.

Book excerpt: The introduction of prostaglandins into clinical practice has been one of the most important advances in obstetrics and gynaecology in recent years. During the last decade obstetricians have become familiar with these drugs for inducing labour and for terminating second-trimester pregnancy. Within the last year their use has been extended, in association with antiprogestin, to first-trimester termi nation. Although the effectiveness of prostaglandins in these pharma cological roles is clear, their full potential has still to be explored, and much remains to be learned about their physiology. Prostaglandins playa central role in the initiation of labour. Further clarification of this role could lead to improvements in preventing or treating preterm labour, which still remains the most important cause of perinatal mortality in this country. Prostaglandins are also inti mately involved in the mechanism of menstruation. More detailed understanding of this role should lead to more effective treatments for menorrhagia and dysmenorrhoea. These substances also playa part in the mechanism of implantation, and further research in this area may lead to more effective therapy for infertility.

The synthesis, metabolism, and function of prosta glandins have been evaluated at the cellular level, in isolated tissues, and/or in the intact organism. The mode of action of prostaglandins and their interactions, particularly with cyclic nucleotides, in normal and pathological conditions, has been discussed with the aim of understanding the possible role of these compounds in hormonal regulation and cell response. The prostaglandin biosynthetic capacity of different tissues, under various experimental conditions, in the presence of specific precursors, and the inhibitory ac tivity of different agents have been examined to ascer tain the relationship between function and metabolism. For these reasons the discussion has also been extended to the methods (biological, immunological, and spectrometric) available for the direct and specific determination of prostaglandins and the evaluation of their synthesis and metabolism.
