Book excerpt: Global fruit and vegetable (F&V) production and consumption are significantly below the threshold needed to meet the FAO and World Health Organization (WHO) recommended F&V intake. In 2017, approximately 4 million premature deaths globally were attributable to insufficient F&V consumption. Globally, very few policy actions specifically promote F&V consumption beyond school-based interventions. Communication and behavior change interventions are insufficient on their own. International evidence suggests that fiscal policies, and policies and programmes that increase access to fresh F&Vs through mobile produce markets can be effective in increasing F&V consumption.

Book excerpt: How we produce and consume food has a bigger impact on Americans' well-being than any other human activity. The food industry is the largest sector of our economy; food touches everything from our health to the environment, climate change, economic inequality, and the federal budget. From the earliest developments of agriculture, a major goal has been to attain sufficient foods that provide the energy and the nutrients needed for a healthy, active life. Over time, food production, processing, marketing, and consumption have evolved and become highly complex. The challenges of improving the food system in the 21st century will require systemic approaches that take full account of social, economic, ecological, and evolutionary factors. Policy or business interventions involving a segment of the food system often have consequences beyond the original issue the intervention was meant to address. A Framework for Assessing Effects of the Food System develops an analytical framework for assessing effects associated with the ways in which food is grown, processed, distributed, marketed, retailed, and consumed in the United States. The framework will allow users to recognize effects across the full food system, consider all domains and dimensions of effects, account for systems dynamics and complexities, and choose appropriate methods for analysis. This report provides example applications of the framework based on complex questions that are currently under debate: consumption of a healthy and safe diet, food security, animal welfare, and preserving the environment and its resources. A Framework for Assessing Effects of the Food System describes the U.S. food system and provides a brief history of its evolution into the current system. This report identifies some of the real and potential implications of the current system in terms of its health, environmental, and socioeconomic effects along with a sense for the complexities of the system, potential metrics, and some of the data needs that are required to assess the effects. The overview of the food system and the framework described in this report will be an essential resource for decision makers, researchers, and others to examine the possible impacts of alternative policies or agricultural or food processing practices.

While there are already commendable efforts in the country to improve overall nutrition knowledge through behavioral change communication, so far, the interventions focus on rural areas and are limited to conveying recommended behaviors with little or no emphasis on the mechanisms by which diet quality can improve nutrition and health outcomes. This experiment will be the first of its kind to introduce systematic nutrition behavioral change communication in urban Ethiopia with an enhanced intervention to communicate the mechanisms through which the consumption of fruits and vegetables can lead to better nutrition and health outcomes.

However, low F&V intake has become a global public health problem. The promotion of F&Vs in the International Year of Fruits and Vegetables emphasizes unprocessed or minimally processed F&Vs, which are highly beneficial to health. However, processing can improve food safety, especially from microbiological contamination, and reduce loss and waste. Conversely, F&Vs which are highly processed can undermine the health benefits of F&V consumption.

Book excerpt: This special issue of Annals of Nutrition and Metabolism presents a series of papers on the first stages of the Pro Children Project. The Pro Children Project was funded by the European Commission to study fruit and vegetable intakes in 11- to 13-year-old children in nine European countries, and to develop and evaluate school-based interventions to promote fruit and vegetable intakes. A multidisciplinary and international team of researchers embarked upon this ambitious evidence-based and theory-driven project. The initial stages reported in this issue include the elaboration of questionnaires, applicable throughout the nine participating countries, for the assessment of fruit and vegetable consumption and potential determinants of consumption in the targeted age group. Secondly, cross-sectional surveys in all nine countries were conducted to assess intake levels and explore determinants in children and their parents. Furthermore, personal and group interviews with children, parents and teachers were conducted. The combined information from these studies was used to design a school-based intervention. The Pro Children Project provides up-to-date intake levels and truly comparable data on fruit and vegetable consumption for the first time. The ongoing evaluation will show if the Pro Children intervention can contribute to healthier diets in European schoolchildren, and will point out the potentials and constraints in implementing school-based intervention strategies across Europe.

Factors that limit the availability and affordability of F&Vs include low production and productivity, the loss of agrobiodiversity, inadequate technology, logistics and infrastructure, weak organizational, business and technical skills, and inefficient market linkages across the supply chain. Many value chains for F&Vs need improvements, investments and shortening as well as gender-responsive approaches, including gender-sensitive investments, to achieve better producer prices, lower risk and higher quality, especially for small farmers.
