In considering the treatment of compound fractures of the skull as met with in civil practice, Sachs3 speaks of the lessons to be learned from the experiences In the war With gunshot wounds of the head, more particularly as to the thorough debridement of the wound. He recognizes, of course, the essential differences between gunshot wounds and the compound fractures of civil life, especially the absence in the latter of imbedded foreign bodies. However, with these and other distinctions, he believes there is an advantage to be gained in the complete excision of traumatized tissue, scalp, dura and brain tissue, in replacing the defect in the dura by a transplanted piece of fascia, and closing the skin incision completely without drainage. Injury of the Longitudinal Sinus. What proved at autopsy to be a laceration of the longitudinal sinus with hemorrhage into the ventricles, following a fall, gave a rather unusual clinical picture as reported by V ine.4 In addition to the subnormal temperature, slow pulse, and unconsciousness, there was intense rigidity of' the entire body, the fists were clenched and the forearms exed across the chest. The knee jerks were exaggerated and the Babinski and Kernig signs were present, with great irritability. The nature of the lesion was not suspected before the operation; at an exploratory craniotomy, the rent in the Sinus was discovered in the neighborhood of the anterior fontanelle and plugged with Horsley's wax. Meningeal Hemorrhages After Contusion of the Skull. The employment of lumbar puncture routinely after cranial injuries of any severity is, I think, good practise. There may be always an element of uncertainty as to the gravity of the lesion and the recovery of blood-stained uid from the spinal canal implies an injury serious enough to have caused a fracture, or meningeal or cortical hemorrhage. The evidence thus secured may be important at least from a medicolegal viewpoint. Guillain5 describes 13 cases in which there was evidence of meningeal hemorrhages following a contusion of the skull, some with, and some without, a scalp wound. In the management of these cases he empha sizes two points: the employment of local anesthesia if an exploratory craniotomy is performed, and the value of repeated lumbar punctures. T 00 much uid should not be withdrawn at first, as the reduction of pressure may cause a secondary hemorrhage. Subsequently repeated puncture is of value in relieving pressure and withdrawing the toxic products of hemolysis. While the symptoms are often vague, Guillain has observed bilateral inversion of the plantar re ex, defensive re exes, some mental confusion and sallowness of the skin. One of these is the pressure to which the food at the pylorus is subjected by the recurring peristaltic waves; the other is the action of the pyloric sphincters. Cannon has shown that gastric peristaltic waves of practically the same intensity are continuously run ning, so long as food remains in the stomach. One of the two factors, the pressure in the antrum, is, therefore, recurrently constant; and the control of the discharge must consequently reside with the other factor, the action of the pyloric sphincter. The substance of Cannon's work is the explanation of this action, as well as the differences in the rate of discharge of different foodstuffs from the stomach. It has been shown that when representative carbohydrate, proteid, and fatty foods of uniform amount and consistency are separately fed to an animal, the carbohydrates begin to leave the stomach within ten minutes after ingestion, and are passed out rapidly. Proteids commonly do not leave the stomach at all during the first half-hour, and sometimes not for an hour, and then they are expelled only slowly. While the last chapter Of trigeminal neuralgia may not have been written, we may go so far as to say that of all the operations on the central nervous system, in none has a greater degree of perfection been obtained and in none are the results so satisfactory. The surgery of the fifth nerve has been dwelt upon so frequently and so extensively in previous volumes of progressive medicine that I scarcely feel justified in discussing the matter again too much at length. There is one phase of this question, however, upon which I have spoken before, and feel justified in speaking of again, namely, the relative gravity of the operation. We have gotten to a point now where we can say, without fear of contradiction, that the mortality of this operation in experienced hands should be less than 3 per cent. In my own series of cases, now numbering 35, there has only been 1 death, the sixth of the series, and I now approach these operations with no more apprehension than any other major procedure. Death in these cases is due to increased intracranial tension acting most forcibly on the nuclei in the region of the fourth ventricle; the amount of uid in the cerebellar fossa is almost always much increased even when there is no obvious compressing agent, such as hemorrhage, foreign body, etc. An attempt should be made to relieve pressure by practising craniectomy in the cerebral fossa on the Opposite side if the injury is in icted in the ante rior part of the head, and on the affected side if the blow was delivered in the occipital region. Of these, 23 patients were between one and ten years of age, 64 between ten and twenty, and 33 twenty years and upward. In 63 the condition was on the right side, in 50 on the left side, and in 7 double. In none of the latter was the failure to descend equal on both sides. With regard to the personal history, it is stated that in 30 cases there were more or less pain and discomfort, and in 9 cases acute attacks of severe pain accompanied by nausea, vomiting, and soreness. In some cases the symptoms simulated torsion of the cord. In the great majority of cases, however, the patient came to the hospital for the treatment of a lump in the groin. In 90 per cent. Of the cases the condition was associated with a hernia. Under the age of ten years there was little change in the size of the undescended testis; after puberty, however, there was usually more or less marked atrophy. It should be noted, however, that the statistics of Bloodgood are based upon Halsted's operation and not Bassini's, a very important point for the reason that in the Halsted method as done at Johns Hopkins Hospital, it was a routine measure to remove a large portion of the veins which undoubtedly was a predisposing cause of orchitis and epididymitis. I can only say that at the Hospital for Ruptured and Crippled, where the veins have not been removed and transplantation of the cord has been done according to the Bassini method, epididymitis, hydrocele or varicocele have been the very rarest occurrences, and they have been seen no more frequently than in the cases in which the cord was not transplanted. The second objection offered is the strengthening of the posterior wall at the expense of the anterior. Connell states that the surgeons who employ the Bassini method, aim to obtain a strong posterior wall or oor. He believes it just as rational to devote the same time and less trouble to making a strong anterior wall. This objection seems of little or no value, inasmuch as the chief object is to cure the hernia, the impartial surgeon caring little whether it be by a strong anterior or strong posterior wall.
